Model for predicting antenatal fetal death up to 28 weeks of gestation

Abstract

Background. Every year, 13.9 stillbirths out of 1000 births, or 2.0 million stillbirths occur worldwide, excluding antenatal losses before 28 weeks of gestation, which some studies estimate would result in an increase of approximately 40%. The stillbirth rate in Russia in 2022 was 5.34 out of 1000 births or 1 in 187 pregnancies, with approximately 20 thousand antenatal losses occurring annually. To date, several attempts have been made to develop a mathematical model for the early detection of pregnancies with a high risk of stillbirth. However, these models do not take into account the gestational age and cannot be applied in resource-limited settings. In this regard, it is important to develop easy-to-use tools for the early detection of pregnancies with a high risk of antenatal fetal death (AFD) depending on the gestational age.

The aim of the study was to develop a mathematical model for predicting AFD at 220–276 weeks of gestation using clinical, anamnestic and laboratory data.

Material and methods. A retrospective study was conducted at the clinical base of the Department of Obstetrics and Gynecology with a course in perinatology of the Medical Institute of the Peoples’ Friendship University of Russia named after Patrice Lumumba in the branch of the antenatal clinic of City Clinical Hospital No. 29 named after N.E. Bauman. Statistical analysis was performed on data from 26 patients with AFD at 220–276 weeks of gestation and data from 30 women with a live birth. At the first stage of the study, an analysis of clinical and anamnestic risk factors for AFD was carried out. At the second stage, the identified predictors were included in the prognostic model of AFD risk.

Results. At the first stage of the study, statistical analysis showed: 1) women with AFD are older than patients with live birth; 2) acute respiratory viral infection with fever and asymptomatic bacteriuria during pregnancy are risk factors for AFD up to 28 weeks of pregnancy; 3) female fetuses are at higher risk of AFD before 28 weeks of gestation compared to male fetuses.

At the second stage of the study, a regression prognostic model was built, including the following parameters: age, BMI, date of registration for pregnancy, acute respiratory viral infection during pregnancy with fever and asymptomatic bacteriuria. The sensitivity of the model was 76.9%, specificity was 90.3%, diagnostic efficiency was 84.2%.

Conclusion. A simple, informative model has been developed for the early detection of pregnant women at high risk of AFD up to 28 weeks of gestation. Research is ongoing to test its performance and effectiveness. A search for additional biomarkers of AFD is also underway.

Вместе с тем в данном исследовании был оценен пол ребенка (табл. 7).

Как видно из табл. 7, девочки чаще подвергались АГП на сроках до 28 нед гестации, чем мальчики (р=0,023).

На втором этапе исследования была выведена прогностическая модель риска АГП на сроках до 28 нед гестации:

где P - вероятность развития АГП на сроках до 28 нед гестации (%); XЛЕТ - возраст (полных лет); XИМТ - индекс массы тела (кг/м2), XУЧ - срок постановки на учет по беременности (полных недель), XОРВИ - ОРВИ с подъемом температуры во время текущей беременности (0 - нет, 1 - да), XББ - бессимптомная бактериурия во время текущей беременности (0 - нет, 1 - да).

Полученная регрессионная модель является статистически значимой (p<0,001). Исходя из значения коэффициента детерминации Найджелкерка модель (1) определяет 64% дисперсии вероятности АПГ на сроках до 28 нед гестации.

Исходя из значений регрессионных коэффициентов, факторы: возраст, ИМТ, срок постановки на учет по беременности, ОРВИ во время беременности с подъемом температуры и бессимптомная бактериурия - имеют прямую связь с вероятностью развития АГП на сроках до 28 нед гестации.

Чувствительность модели составила 76,9%, специфичность - 90,3%, диагностическая эффективность - 84,2%.

ROC-анализ показал высокую прогностическую значимость полученной регрессионной модели (см. рисунок).

Площадь под ROC-кривой, соответствующей взаимосвязи прогноза АГП на сроках до 28 нед беременности и значения логистической регрессионной функции, составила 0,903±0,042 с 95% ДИ 0,82-0,99.

Пороговое значение функции P (1) в точке cut-off составило 0,6. Значения функции, равные или превышающие данное значение, соответствовали прогнозу АГП. Чувствительность и специфичность метода составили 70,2 и 93,5% соответственно.

Обсуждение

В этом исследовании мы разработали простую в использовании модель прогнозирования беременности с высоким риском АГП на сроках до 28 нед гестации. Прогностическая модель включает в себя 6 легко и недорого измеряемых доступных параметров, что способствует ее использованию во время дородовых визитов в условиях ограниченных ресурсов женской консультации.

К настоящему времени было проведено небольшое количество исследований, касающихся разработки инструмента для прогнозирования риска мертворождения, а модели, стратифицированные по гестационному сроку, вовсе не разрабатывались. Так, А. Koivu и соавт. в своей модели смогли достичь диагностической эффективности в 73-74%, используя клинико-анамнестические данные пациенток, в то время как Е. Malacova и соавт. достигли максимальной точности в 84% на основании таких же параметров [6, 7]. В нашем исследовании получена математическая модель, имеющая аналогичную диагностическую эффективность - 84,2%. Однако ни в одном из вышеуказанных исследований не учитывался гестационный срок на момент АГП. Прогностическая модель Е. McPherson и соавт. включала ультразвуковые показатели, полученные по результатам первого скрининга, однако данный инструмент невозможно использовать у пациенток, ставших на учет в женскую консультацию во II триместре беременности [8].

Т. Khatibi и соавт. использовали сложный выбор признаков (с помощью кластерного анализа K-средних) для определения предикторов АГП и ввода их в классификаторы машинного обучения [9]. Несмотря на то что авторы смогли достичь высокой точности модели (до 90,6%), они использовали такой показатель, как гестационный возраст на момент родов, что не является клинически значимым для прогнозирования антенатальных потерь. В то же время мы стремились выявить пациенток с риском АГП во II триместре, используя данные, доступные до родов, вследствие чего наша модель дает возможность изменить тактику ведения беременных на более ранних этапах.

Однако, прежде чем нашу или другие прогностические модели можно будет использовать в клинической практике, их следует проверить проспективно. Если в проспективных исследованиях сохранится высокий риск развития АГП, могут быть разработаны практические рекомендации с использованием данных моделей, а для удобства разработаны калькуляторы.

Полученная прогностическая модель также имеет несколько ограничений. Для разработки модели была использована небольшая когорта, что снижает мощность исследования. Кроме того, наша модель не включала оценку биомаркеров и результаты ультразвукового исследования, что смогло бы повысить диагностическую точность. В модели, разработанной R. Akolekar и соавт., наряду с изучением клинико-анамнестических данных пациенток оценивали уровни биомаркеров плацентарных факторов роста (placental growth factor, PlGF) и допплерографии в I и II триместрах беременности, что увеличило диагностическую точность инструмента на 16% [10].

Заключение

Разработана простая информативная модель для раннего выявления беременных с высоким риском АГП на сроках до 28 нед беременности. Продолжаются исследования для проверки ее производительности и эффективности, а также поиск дополнительных биомаркеров АГП.
