Clinical and serological characteristics of women with non-developing pregnancy and obesity

Abstract

Background. Obesity is a multifactorial disease caused by a combination of environmental, psychosocial factors and genetic predisposition. According to the World Health Organization, more than 890 million adults worldwide were obese in 2022. A number of studies demonstrate that in obese patients the risk of reproductive losses increases by 25–37% compared with normal weight women.

The aim of the study was to investigate the proteomic profile of women with reproductive losses due to obesity.

Material and methods. The study was conducted at the clinical base of the Department of Obstetrics and Gynecology with a course of perinatology at the Medical Institute of the Patrice Lumumba Peoples’ Friendship University of Russia – City Clinical Hospital named after V.M. Buyanov. 130 patients with reproductive losses were examined and divided into 2 groups. The main group included obese women (n=69), the control group included patients with normal body weight (n=61).

Conclusion. Statistical analysis showed that the following proteins are promising serological markers for predicting failure to develop pregnancy in obesity: plasma protease inhibitor 1C, retinol binding protein 4 and transthyretin.

При сравнении уровней белка RET4 у женщин с НБ и ожирением и у нормовесных пациенток с НБ были установлены статистически значимые различия (p<0001*) (см. таблицу). Уровень белка был существенно выше у женщин с ожирением по сравнению с нормовесными пациентками (медианы составляли 541,8 и 425,3 соответственно).

При сравнении уровней белка TTHY у женщин с НБ и ожирением и у нормовесных пациенток с НБ были также установлены статистически значимые различия (p=0001*) (см. таблицу). Уровень TTHY был существенно выше у женщин с ожирением по сравнению с пациентками с нормальным ИМТ (медианы составляли 1817,9 и 1361,0 соответственно).

Различия в уровнях белка ANGT в зависимости от ИМТ пациенток были статистически незначимыми (p=0,345) (см. таблицу).

Обсуждение

В процессе исследования были выявлены серологические маркеры, имеющие потенциальное значение в прогнозировании НБ у женщин с ожирением.

В нашем исследовании продемонстрировано, что существует прямая связь между НБ у женщин с ожирением и циркулирующими серологическими маркерами в сыворотке крови. Полученные результаты согласуются с данными зарубежных исследований. Так, S. Cheng и соавт. (2023) продемонстрировали, что повышение уровня TTHY в сыворотке крови увеличивает риск репродуктивных потерь при ожирении [10].

Результаты исследования свидетельствуют о том, что более высокая концентрация белка IC1 характерна для пациенток с НБ и ожирением. Полученные данные схожи с результатами зарубежных авторов. N. Gabriel и соавт. (2022) и E. Karabiber и соавт. (2022) в своих исследованиях показали, что у женщин с ожирением и повышенным уровнем белка IC1 чаще отмечалось НБ, чем у нормовесных пациенток [16, 17].

Кроме того, результаты настоящего исследования соответствуют данным, полученным L. Czuba и соавт. (2022), показавшим, что повышенная концентрация белка RET4 приводит к репродуктивным потерям чаще у женщин с ожирением по сравнению с пациентками с нормальным ИМТ [14].

Заключение

В данном исследовании продемонстрировано 3 потенциальных серологических маркера, ассоциированных с репродуктивными потерями при ожирении. Предикторами НБ при ожирении могут служить IC1 (p<0,0001), RET4 (p<0,0001), TTHY (p<0,0001). Пороговыми уровнями данных биомаркеров НБ при ожирении считаются: 3053,6 для IC1 (p<0,0001), 541,8 для RET4 (p<0,0001) и 1817,9 для TTHY (p<0,0001). Полученные данные подчеркивают необходимость проведения дальнейших исследований, направленных на изучение протеома у беременных с ожирением.
