Clinical case of non-traumatic fracture of the femoral neck and spine in a woman in labor with osteogenesis imperfect –Lobstein–Van Der Hoeve syndrome

Abstract

Osteogenesis imperfecta constitutes a complex connective tissue disorder extending beyond its hallmark bone fragility. Van der Hoeve’s syndrome is a variant of Lobstein’s disease (late osteogenesis imperfecta) and is characterized by the simultaneous presence of osteoporosis, blue sclerae, and deafness. We are presenting a case of postpartum atraumatic fracture of the femur and spine in a patient in the late postpartum period. The cause of the fractures was the presence of osteogenesis imperfecta with an autosomal dominant type of inheritance.

На 54-е сутки после родов по результатам МРТ правого тазобедренного сустава был диагностирован закрытый патологический базальный перелом шейки правой бедренной кости без смещения обломков. Врачом-травматологом рекомендован прием препаратов кальция и опора на трость при ходьбе.

Пациентке была проведена рентгеновская денситометрия (через 3 мес после родов): Т(Z)-критерий был равен -4,2 в поясничном отделе позвоночника и -3,8 в шейке правого бедра (см. рисунок). Следует отметить, что остеопороз диагностируется при Т(Z)-значении критериев меньше 2,5. Таким образом, образом у 37-летней пациентки был впервые диагностирован тяжелый остеопороз поясничного отдела позвоночника и шейки бедра.

Через 5 мес после родов после подъема тяжести (детская коляска) у пациентки произошел компрессионный перелом поясничного отдела позвоночника.

По результатам генетического консультирования панели было установлено заболевание соединительной ткани. Выявлен вариант, являющийся наиболее вероятной причиной заболевания: chr 17:5086639С>Т, носительство мутации в гене COL1A1, кДНК-сю3814+1G>A в гетерозиготном состоянии (заболевание, ассоциированные с геном, несовершенный остеогенез с аутосомно-доминантным типом наследования). Остеопороз с тяжелым прогрессирующим течением.

В настоящее время пациентка получает препараты кальция и витамина D, планируется терапия бисфосфонатами.

Обсуждение

Несовершенный остеогенез - редкое генетическое заболевание, связанное с хрупкостью костей. Пациенты с несовершенным остеогенезом обычно страдают от многочисленных переломов, деформаций скелета, низкорослости и потери слуха. Расстройство характеризуется генетической и клинической гетерогенностью. Патогенные варианты более чем в 20 различных генах могут приводить к несовершенному остеогенезу, а фенотипы могут варьировать от легких до летальных форм [19, 20].

Клиническим признаком несовершенного остеогенеза является склонность к переломам из-за снижения прочности кости. Беременность и кормление грудью являются периодами повышенной потребности в кальции, что приводит у женщин с несовершенным остеогенезом к дополнительному снижению минеральной плотности костной ткани [8, 21].

Ломкость костей при синдроме Лобштейна-Ван-дер-Хеве может появляться в различные сроки, исходя из чего различают 3 вида болезни. Первые 2 варианта наиболее тяжелые, при которых переломы появляются внутриутробно, в период родов или в раннем детстве. Третий вариант (поздний) характеризуется появлением переломов в возрасте 2-3 лет; количество и опасность их возникновения со временем уменьшается к пубертатному периоду [16]. Во многих случаях отмечается относительно доброкачественное течение заболевания - после 20-летнего возраста ломкость костей становится более умеренной.

Клинические обследования больных с синдромом Лобштейна-Ван-дер-Хеве выявляют ряд характерных признаков заболевания - истончение склер, уменьшение количества коллагеновых, а также эластических волокон, метахроматический окрас главной субстанции склер, который обусловлен высоким содержанием мукополисахаридов, что указывает на незрелость фиброзной ткани, персистенцию эмбриональной склеры. Вторым главным клиническим проявлением синдрома "голубых склер" является высокая ломкость костей в сочетании с особой слабостью связочного аппарата и суставов. Эти признаки выявляются почти у 65% пациентов с данным синдромом на разных сроках течения патологического процесса [15].

В представленном нами наблюдении у пациентки с учетом особенности ее анамнеза вероятнее всего имел место третий вариант (поздний) синдрома Лобштейна-Ван-дер-Хеве. К тому же данная триада симптомов отмечалась у ее матери, что также подтверждает генетический характер заболевания.

В представленных в литературе отчетах о клинических случаях подчеркивается, что в послеродовом периоде постоянная боль в нижних конечностях может указывать на повреждения костей, такие как стрессовые переломы, особенно развивающиеся вскоре после интенсивной физической нагрузки [21]. В то же время переломы костей нижних конечностей и компрессионные переломы позвоночника во время беременности и в послеродовом периоде чаще всего встречаются у женщин с несовершенным остеогенезом.

Заключение

Проблеме осложнений течения беременности и послеродового периода у женщин с несовершенным остеогенезом в отечественных источниках литературы уделено мало внимания. Вместе с тем врачи акушеры-гинекологи должны иметь знания не только о наличии таких заболеваний, но и быть готовыми к проведению адекватных лечебно-диагностических мероприятий, своевременно консультировать таких пациенток у специалистов соответствующего профиля. Лечение пациенток с несовершенным остеогенезом во время беременности и после родов должно быть направлено на оптимальный контроль модифицируемых факторов риска остеопороза, особенно у пациенток с низкой минеральной плотностью костной ткани. Таким пациенткам рекомендуется избегать грудного вскармливания, поскольку лактация является фактором, негативно влияющим на минеральную плотность костной ткани.
