Pathogenesis and prediction of recurrent endometrial polyps

Abstract

Endometrial polyps are a common pathology that, despite often being asymptomatic, is associated with reproductive disorders. Hysteroscopic resection of polyps is an effective treatment method, but recurrences are not uncommon. The recurrence rate after polypectomy varies from 13 to 43% according to various studies. Predisposing factors include characteristics such as hyperplastic type of polyps, a greater initial number of polyps, and the duration of follow-up. A detailed analysis of the hysteroscopic characteristics of the endometrium and the histological features of the removed polyps will optimize the management of patients after polypectomy for the timely detection and treatment of recurrences.

Aim– to identify morphological and hysteroscopic predictors of endometrial polyp recurrence in patients of reproductive age.

Material and methods. A prospective cohort comparative open study included 190 patients of childbearing age diagnosed with endometrial polyps. The age of the participants ranged from 28 to 43 years, with a median value of 36.96 years (interquartile range – 33.997–39.086). Medical history and anthropometric data were collected, sonographic examination of the pelvic organs, hysteroscopy of the uterine cavity with targeted endometrial biopsy, morphological and immunohistochemical studies were performed. Statistical analysis with odds ratio calculation was conducted.

Results. Significant (p<0.05) morphological predictors of endometrial polyp recurrence are: CD138 expression greater than 5 (OR 5.319), hyaline thickening of polyp vessels (OR 4.049), presence of collagen swirls in the glands (OR 3.830), diffuse stromal fibrosis (OR 3.154), CD20 expression greater than 0 (OR 2.786), and sclerosis of spiral arteries (OR 2.213). Significant (p<0.05) hysteroscopic predictors of endometrial polyp recurrence are: diffuse hyperemia (OR 5.120; 95% CI 2.224–11.788), vascularization (OR 5.135; 95% CI 2.131–12.372), micropolyposis (OR 4.625; 95% CI 1.799–11.891), stromal edema (OR 2.933; 95% CI 1.099-7.828), focal hyperemia (OR 2.321; 95% CI 1.201–4.485), and a broad polyp base (OR 1.936; 95% CI 1.039–3.607).

Conclusion. Chronic endometritis may contribute to the recurrence of endometrial polyps.

Частота сочетания ХЭ и ПЭ активно изучается в гинекологии. По данным обзорных исследований, ХЭ обнаруживается более чем у половины пациенток с ПЭ [19]. Предполагается, что ХЭ и ПЭ могут быть последовательными этапами единого патологического процесса на фоне хронического воспаления [12]. Сходство нарушений микробиома полости матки при ХЭ и ПЭ также подтверждает общность этиологии этих состояний [19-21]. Таким образом, имеется тесная патогенетическая связь между полипозной трансформацией эндометрия и ХЭ [19-23].

Этиопатогенез ПЭ долгое время рассматривался как результат нарушения рецепции ткани эндометрия к эстрогену [24]. Однако некоторые ПЭ часто рецидивируют даже после надлежащей гормональной терапии, а это указывает на то, что в этиологии ПЭ задействованы и другие факторы. Аккумулируется все больше доказательств в пользу того, что воспалительный компонент в патогенезе полипозной трансформации следует рассматривать по крайней мере у некоторых подтипов полипов [25]. Классические факторы, связанные с воспалением, включая NF-κB (ядерный транскрипционный фактор), IFNγ (интерферон γ) и IL-17 (интерлейкин-17) [26-28], активируются в полипах наряду с изменениями сосудов, вторичными также по отношению к эндометриту [29]. Локальные нарушения в клеточной иммунной среде, наблюдаемые при ПЭ, такие как избыточная активность тучных клеток и повышенная экспрессия c-Kit, также свидетельствуют о том, что ПЭ являются следствием персистенции локального воспалительного процесса [30]. В последнее время исследователи высказывают предположение о том, что у женщин с ПЭ выше вероятность ХЭ в сравнении с пациентками без ПЭ [31].

ПЭ часто приводят к нарушениям менструального цикла, бесплодию и привычному невынашиванию [9, 11, 32]. "Золотым стандартом" лечения ПЭ в настоящее время считается гистерорезектоскопия [32, 33]. Однако частота рецидивов через 12 мес после полипэктомии достигает 43% [6, 7]. Факторы риска рецидивирования ПЭ после полипэктомии до конца не ясны. Из известных факторов риска количество ПЭ и предыдущее оперативное лечение практически не поддаются коррекции, поэтому необходимо изучать потенциальную роль модифицируемых факторов, например ХЭ. Учитывая участие ХЭ в формировании ПЭ, логично предположить, что сохраняющийся или рецидивирующий воспалительный процесс после полипэктомии может способствовать рецидивам заболевания.

В нашем исследовании мы проанализировали гистероскопические характеристики ПЭ, а также морфологические и ИГХ-особенности биоптатов пациенток с рецидивирующими ПЭ после гистерорезектоскопической полипэктомии.

Анализ показал, что для рецидивирующих полипов характерны более высокие уровни экспрессии иммуногистохимических маркеров CD138 и CD20 по сравнению с группой первичных полипов. Кроме того, в группе рецидивирующих полипов достоверно чаще наблюдались гистологические признаки фиброзно-склеротической трансформации эндометрия: отложения коллагена в железах, диффузный фиброз стромы, склерозирование спиральных артерий и гиалиновое утолщение сосудов полипа.

При сравнительном анализе гистероскопических характеристик в группе пациенток с рецидивирующими полипами также была выявлена более высокая частота встречаемости признаков хронического воспаления. К тому же полипы в группе рецидива имели бóльшие размеры по сравнению с первичными полипами, что может свидетельствовать о более агрессивном росте при рецидивах. Кроме того, при рецидивирующих полипах значимо чаще выявлялись такие сопутствующие изменения эндометрия, как микрополипоз и отек стромы, что указывает на более выраженные системные нарушения микроокружения и архитектоники эндометрия в этой группе. Анализ локализации полипов показал, что при рецидивах они преимущественно располагались на дне матки, в то время как первичные полипы чаще локализовались на задней стенке. Данный факт может иметь практическое значение при выборе тактики лечения и последующего наблюдения за пациентками. Рецидивирующие полипы характеризовались более выраженной васкуляризацией и чаще имели широкое основание по сравнению с первичными полипами. Эти особенности могут отражать более интенсивный неоангиогенез и инфильтративный рост при рецидивах.

Полученные результаты подтверждают гипотезу о тесной патогенетической связи между развитием ХЭ и рецидивированием эндометриальных полипов после хирургического лечения. Повышенная экспрессия маркеров В-лимфоцитов и плазматических клеток (CD20 и CD138), а также наличие выраженных признаков фиброзно-склеротической трансформации эндометрия могут рассматриваться в качестве предикторов рецидивирования заболевания.

Заключение

В результате проведенного исследования установлено, что для рецидивирующих эндометриальных полипов после хирургического лечения характерна более высокая экспрессия маркеров В- и плазматических клеток (CD20 и CD138), а также более выраженные признаки фиброзно-склеротической трансформации ткани эндометрия по сравнению с первичными полипами, что свидетельствует о тесной патогенетической связи между ХЭ и рецидивированием заболевания и обосновывает необходимость назначения комплексной терапии, включающей лечение хронического воспаления наряду с хирургическим удалением полипов, для профилактики рецидивов.

Достоверными (p<0,05) морфологическими предикторами рецидивирования ПЭ являются экспрессия CD138 >5 клеток в поле зрения (ОШ 5,319), гиалиновое утолщение сосудов полипов (ОШ 4,049), наличие коллагеновых завихрений в железах (ОШ 3,830), диффузный фиброз стромы (ОШ 3,154), экспрессия CD20 >0 клеток в поле зрения (ОШ 2,786), склерозирование спиральных артерий (ОШ 2,213).

Достоверными (p<0,05) гистероскопическими предикторами рецидивирования ПЭ являются диффузная гиперемия (ОШ 5,413; 95% ДИ 2,332-12,561), васкуляризация (ОШ 5,394; 95% ДИ 2,221-13,100), микрополипоз (ОШ 4,859; 95% ДИ 1,878-12,575), отек стромы (ОШ 3,077; 95% ДИ 1,148-8,264), очаговая гиперемия (ОШ 2,456; 95% ДИ 1,265-4,766) и широкое основание полипа (ОШ 2,060; 95% ДИ 1,103-3,850).
