Diagnosis of premature rupture of membranes in pregnant women with isthmic-cervical insufficiency using the IGFBP-1 PROM rapid test
AbstractThe efficacy evaluation of the IGFBP-1 PROM rapid test was perfomed in 157 pregnant women with symptoms and signs of premature rupture of membranes. The results obtained showed that the IGFBP-1 immunoassay has high sensitivity and specificity.
The aim of the research was to diagnose premature rupture of fetal membranes in pregnant women with an obstetric pessary or cerclage using the IGFBP-1 PROM rapid test in case of suspected leakage of amniotic fluid.
Material and methods. The study included 157 women with singleton pregnancies at high risk of preterm delivery due to isthmic-cervical insufficiency (cervical length ≤25 mm and/or cervical infundibulum) or a history of preterm delivery.
Results. The rapid test is an accurate bedside test for diagnosing premature rupture of fetal membranes in women with threatened preterm labor. It has a sensitivity of 97.6%, a specificity of 93.1%, a positive predictive value of 98.4%, a negative predictive value of 90.0% and an overall accuracy of 96.8% in diagnosing premature rupture of fetal membranes.
Conclusion. IGFBP-1 immunoassay is an immunologic rapid test for the diagnosis of premature rupture of fetal membranes with high sensitivity and specificity.

Статистический анализ проведен с помощью пакета прикладных программ Statistica 10.0. Математико-статистическое описание объекта исследования осуществлялось с помощью расчета средних значений показателей (среднего арифметического значения, моды, медианы). Значимость различия средних значений анализировали с помощью t-критерия Стьюдента. Различия признавали статистически значимыми при p<0,05.
Чувствительность рассчитывали по следующей формуле: количество истинно-положительных результатов (ИП) / [ИП + количество ложноотрицательных результатов (ЛО)]; специфичность: количество истинно-отрицательных результатов (ИО) / [ИО + количество ложноположительных результатов (ЛП)]; прогностичность положительных результатов (ППР): ИП / (ИП + ЛП); прогностичность отрицательных результатов (ПОР): ИО/(ИО + ЛО).
Кроме того, для каждого теста дополнительно оценивали точность: (ИП + ИО) / (ИП+ ЛО + ЛП + ИО) × 100%.
Результаты
Сравниваемые группы беременных не отличались по возрасту и антропометрическим параметрам. Течение беременности на ранних сроках у женщин всех групп характеризовалось высокой частотой угрозы прерывания беременности и ИЦН. Значимых различий в частоте развития осложнений между группами не наблюдали.
Всего в исследование были включены 157 беременных со средним возрастом 24,8±4,5 года, средний срок беременности - 32,6±3,2 нед. В 1-й группе отягощенный акушерско-гинекологический анамнез был у 52 (65%) беременных, во 2-й группе - у 55 (71,5%) (p<0,05).
По данным цервикометрии длина шейки матки колебалась в диапазоне от 1,7 до 2,8 см, составляя в среднем 2,3±0,26 см.
Среди пациенток, которым был выполнен экспресс-тест, диагноз ПРПО был поставлен в 6 (3,8%) случаях.
В группе с использованием акушерского пессария в одном случае произошло преждевременное излитие околоплодных вод на 35-й неделе беременности и у 2 (2,5%) беременных в сроке 36 нед. В группе с использованием серкляжа преждевременное излитие околоплодных вод произошло у 10 (12,9%) беременных в сроках 32-36 нед беременности (p<0,05).
Частота оперативных родов (кесарево сечение) в 1-й группе составила 8 (10%), во 2-й группе - 20 (25,9%) беременных (p<0,02).
Экспресс-тест является высокоточным прикроватным тестом для диагностики ПРПО у женщин с угрожающими ПР. Он обладает чувствительностью 97,6%, специфичностью 93,1%, положительной прогностической ценностью 98,4%, отрицательной прогностической ценностью 90,0% и общей точностью 96,8% (см. таблицу).

Заключение
Иммуноанализ на IGFBP-1 (IGFBP-1 PROM TEST) представляет иммунологический экспресс-тест для диагностики ПРПО. Согласно результатам проведенного исследования, данный экспресс-тест обладает чувствительностью 97,6%, специфичностью 93,1%, положительной прогностической ценностью 98,4%, отрицательной прогностической ценностью 90,0% и общей точностью 96,8%.
В данном исследовании неинвазивная оценка с применением экспресс-теста IGFBP-1 PROM TEST позволила довольно точно определить наличие истинного ПРПО, а также выявить женщин с ложными признаками ПРПО, которые не родили в течение ближайших 7 дней.
Использование данного теста в клинической практике поможет снизить существенные затраты, связанные с ненужной госпитализацией, но оно не исключает проведения дополнительных методов исследования.
