Modern risk paradigm for atypical endometrial hyperplasia: predictive capabilities

Abstract

The aim was to summarize modern ideas about risk factors, clinical picture, possibilities of early diagnosis and personal prognosis of the risk of atypical endometrial hyperplasia.

Material and methods. An analytical review of scientific publications in scientometric databases (Pubmed, e-library) for the period since 2017 until 2023 was carried out.

Conclusion. A promising direction of scientific research is the development of methods for personalized prediction of the risk of developing AEH using a maximum of personal parameters, including anamnestic data, clinical manifestations, results of the use of imaging techniques, as well as assessment of the expression of IHC markers reflecting the proliferative potential of the tissue.

В 2020 г. E. Barnas и соавт. первыми провели исследование, в котором использовали инфракрасную спектроскопию с преобразованием Фурье (FTIR) и рамановскую спектроскопию в сочетании с многомерным анализом для идентификации и дифференциации тканей РЭ и АГЭ. В качестве контроля исследовали нормальный эндометрий. В результате спектры FTIR (Fourier transform infrared spectroscopy, инфракрасная спектроскопия на основе преобразования Фурье) показали выраженное снижение максимальной оптической плотности (МОП), соответствующей нуклеиновой кислоте. Был отмечен факт, что снижение МОП усиливалось по мере перехода поражения от АГЭ к РЭ. Получены сведения о том, что рамановская спектроскопия (названа в честь индийского физика Ч.В. Рамана. - Примеч. ред.) позволяет лучше дифференцировать нормальные ткани от подверженных атипии и онкотрансформации, что может быть связано с их структурой и симметрией функциональных групп, образующих исследуемую ткань [34].

Интересные сведения были получены при выполнении биоинформационного анализа с использованием базы данных UALCAN, который показал, что мРНК и белок YTHDF2 активируются при РЭ и более высоко экспрессируются при РЭ и АГЭ по сравнению с нормальными тканями эндометрия, ГЭ без атипии и аденомиозом. Интересно, что белок YTHDF2 не только помогает различить РЭ и АГЭ, но и может быть полезен для точной оценки глубины инвазии миометрия при РЭ, когда он возникает на фоне аденомиоза, так как аденомиоз могут ошибочно принять за инвазию опухоли в миометрий [35].

Заключение

Несмотря на многочисленные исследования, посвященные проблематике ГЭ, патогенез и персональный прогноз АГЭ остаются "проблемой внутри проблемы", далекой от своего решения. Данные официальной статистики неумолимо свидетельствуют о том, что современная медико-демографическая ситуация в России характеризуется преобладанием в общей популяции женщин, потенциально имеющих риск не только ГЭ, но и АГЭ и даже РЭ, в том числе в репродуктивном возрасте [8].

Исследования в отношении возможности стратификации женского населения по риску АГЭ до сих пор были преимущественно ориентированы на пациенток пери- и постменопаузального периодов жизни.

Рутинное выделение групп риска ГЭ не всегда дает точный прогноз предпосылок к атипической трансформации ткани эндометрия. В действующих российских клинических рекомендациях [18] не определены показания к ИГХ-исследованию с целью дифференциальной диагностики ГЭ, АГЭ и РЭ, что подтверждает необходимость дальнейшего научного поиска в этом направлении. Повышение результативности ранней диагностики и прогнозирования персонального риска рецидивов и прогредиентного течения ГЭ и АГЭ, рассчитанного на основе доказанных предикторов, включая медико-социальные характеристики, клинические проявления, данные, полученные с помощью визуализирующих методик, а также оценку экспрессии ИГХ-маркеров, отражающих пролиферативный потенциал ткани, является перспективным направлением научного поиска и будет иметь важное значение для клиницистов и организаторов здравоохранения.
