Features of early reproductive losses in obese women
AbstractObesity, being the main non-infectious pandemic of the 21st century, is associated with a wide range of obstetric and perinatal complications. The problem of early pregnancy in patients with excess adipose tissue is the high frequency of early reproductive losses in the absence of genetic disorders of the embryo. The search for the causes of miscarriage in this group of patients is an urgent problem in modern obstetrics. This article discusses the main pathogenetic mechanisms of early reproductive losses in obesity.

В ряде исследований выявлено, что у женщин с ожирением и привычным невынашиванием беременности частота аномальных аллелей гена прогестерона [1031С, 1978Т, 2310Т/С(PROGINS)] выше [22, 23]. На иммунокомпетентных клетках под воздействием прогестерона экспрессируется прогестерон-индуцированный блокирующий фактор, он блокирует продукцию провоспалительных цитокинов ИЛ-1, ИЛ-6, ФНОα и увеличивает продукцию провоспалительных цитокинов ИЛ-10, ИЛ-13. При недостаточной продукции прогестерона смещение в сторону провоспалительных цитокинов может сопровождаться локальным тромбообразованием, препятствующим нормальной имплантации и инвазии трофобласта [15, 23-25].
Заключение
Перечисленные выше изменения, ассоциированные с ранними потерями беременности у пациенток с ожирением, свидетельствуют о необходимости проведения прегравидарной подготовки у данного контингента женщин с целью минимизации рисков и повышения шансов на успешные исходы беременностей.
