The role of microRNAs in the pathogenesis of recurrent uterine leiomyomas
AbstractThe aim of the study was to determine the epigenetic determinants of recurrent uterine leiomyoma at reproductive age patients.
Material and methods. The study included 66 patients who were divided into two groups: I – 42 patients who underwent myomectomy, of which 18 (42.9%) had a history of relapse; II – 24 who underwent hysterectomy, of which 4 (16, 7%) with a history of relapse. The patients underwent treatment and observation in the gynecology department at City Clinical Hospital # 29 named after N.E. Bauman. A section of tissue measuring 5×5 cm from leiomyoma nodes and myometrium was cut out from the surgical material in case of hysterectomy. The cut tissue was frozen at -25 °C. The expression of 21 microRNAs was studied: let7, 20a-3p, 23b, 29b, 146b-5p, 92a-1-5, 93-3p, 100-3p, 10b, 223-3p, 182-5p, 146b-5p, 21-5p, 200a-3p, 30a-3p, 156a-5p, 122-5p, 10a-5p, 15a, 16, 26a. MicroRNA expression levels were measured by real-time PCR using a CFX96 amplifier (Bio-Rad Laboratories, USA). Analysis of the obtained real-time PCR threshold cycle data was carried out using the 2(–∆Ct) method (Livak and Schmittgen, 2001).
Statistical processing of the obtained data was carried out on the basis of the nonparametric Mann–Whitney coefficient using STATISTICA 12.0 software (StatSoft Inc., USA). If p<0.05, the difference was considered statistically significant
Design. The prospective non-randomized longitudinal study was performed.
Results. The groups were comparable in age at menarche and onset of sexual activity, frequency of previous and concomitant extragenital and gynecological diseases. Myomectomy was performed in younger patients 39.6 years old (37.9–41.2) compared to the group of patients with hysterectomy – 46.8 years old (46.0–47.5) with a statistically significant difference p=0.001. When studying the parity of patients in groups I and II, statistically significant differences were also revealed (p=0.021). In reproductive age, patients with infertility underwent myomectomy significantly more often compared to patients with a history of spontaneous childbirth (p=0.032). When comparing the duration of the disease depending on the volume of surgical treatment, no statistically significant differences were found. At the same time, myomectomy was performed on every fourth patient with a uterine size up to 10 weeks of pregnancy (38.1%), every second from 11 to 15 weeks of pregnancy (50.0%) and 11.9% – more than 15 weeks of pregnancy, and in the group of patients operated on in the scope of hysterectomy in 4.2%; 66.7% and 29.2% respectively, p=0,006. When comparing the size of the uterus depending on the volume of surgical treatment, statistically significant differences were obtained (p=0.005).
Every second patient who underwent myomectomy [n=18 (42.9%)] has a history of reccurence of the disease occurred within 1–3 years, and every sixth person who underwent hysterectomy had a history of relapse of the disease [n=4 (16.7%)]. When analyzing the parity of patients with recurrent UL and without relapse, statistically significant differences were obtained (p=0.012). It was found that patients without a history of childbirth have a higher probability of recurrent UL compared to patients with a history of spontaneous childbirth (p=0.023). There was a relatively strong connection between the compared characteristics (Cramer’s V = 0.360). Statistically significant differences were also established in the duration of the disease in the compared groups (with and without relapse, p=0.023).
When comparing the level of microRNA expression in myomatous nodes depending on the volume of surgical treatment (myomectomy or hysterectomy), no statistically significant differences were revealed. However, when comparing the expression of microRNAs in myomatous nodes and myometrium in the material after hysterectomy, statistically significant differences were obtained for the following 8 microRNAs out of 20: let7, 20a, 26a, 29b, 100, 10b, 21-5p, 10a-5p. The expression of Mir Let7, Mir 20a, Mir 100, Mir 10a-5p was one and a half times lower in the myomatous node compared with similar indicators in the myometrium, and Mir 26a, Mir 29b, Mir 10b – two times lower. As for Mir 21-5p, the expression of this microRNA was 1.6 times higher in the myomatous node compared to that in the myometrium.
Conclusion. The identified differences in the expression of 8 microRNAs out of 20 studied: let7, 20a, 26a, 29b, 100, 10b, 21-5p, 10a-5p prove that these nucleotide sequences play a key role in the formation of UL. This provides incentive for further in-depth research in this area. We have not identified a relationship between the level of microRNA expression in myomatous nodes depending on the volume of surgical treatment (myomectomy or hysterectomy) and in the comparison groups depending on the history of relapse. On the contrary, a decrease in the expression levels of Mir Let7, Mir 20a, Mir 100, Mir 10a-5p was detected by one and a half times in the myomatous node, compared with similar indicators in the myometrium, and Mir 26a, Mir 29b, Mir 10b – by a factor of two. As for Mir 21-5p, the expression of this microRNA was 1.6 times higher in the myomatous node compared to that in the myometrium.

Избыточное накопление и ремоделирование являются ключевыми механизмами в формировании фибротических заболеваний. Клетки ЛМ характеризуются избыточным накоплением неорганизованного ВКМ. В ЛМ представлены глюкозаминогликаны и интерстициальный коллаген. Предполагается, что богатая ВКМ ригидная структура становится причиной аномальных маточных кровотечений и тазовых болей. Накопление ВКМ также связано с влиянием факторов роста: трансформирующего фактора роста b (ТФР b; Transforming growth factor, TGFb), активина-А, тромбоцитарного фактора роста (ТРФ), цитокинов фактора некроза опухоли a (Tumor necrosis factor, TNFa), стероидных гормонов (эстрогена, прогестерона) и миРНК.
Формирование опухолей не может быть полностью объяснено генетическими дефектами, имеют место также и эпигенетические факторы. К ним относятся метилирование ДНК, модификация гистонов, некодирующие РНК, гетерохроматизация. Изменения в экспрессии следующих микроРНК описаны в литературе: let7, миРНК-21, миРНК-93, миРНК-106b, миРНК-29, миРНК-200. миРНК длиной от 19 до 25 нуклеотидов представляет некодирующую малую РНК, обнаруженную в клетках эукариот, которая может регулировать процесс развития и участвует в важном механизме посттранскрипционной регуляции эукариотических клеток [1]. Исследования показали, что миРНК, составляющая менее 3% генома человека, регулирует до 30% генов. Кроме того, более 50% миРНК расположено в регуляторной области генов, связанных с опухолью, поэтому дисбаланс миРНК, вероятно, является одним из важных механизмов развития опухоли [2]. В последние годы все больше исследований показывают, что миРНК можно использовать в качестве потенциального диагностического онкомаркера.
В исследовании R. Lazzarini и соавт. при сравнении нормального миометрия с клетками ЛМ обнаружено 7 миРНК со сниженной экспрессией (миРНК-146b-5p; миРНК-335-3p; миРНК-335-5p; миРНК-135b-5p; миРНК-10a-3p; миРНК-10a-5p; миРНК-200a-3p) и 8 микроРНК с повышенной экспрессией (миРНК-146a-5p; миРНК-576-3p; миРНК-122-5p; миРНК-1246; миРНК-595; миРНК-658; миРНК-4284; миРНК-924). При помощи компьютерной программы DIANA-miRNAPath авторы определили, что вышеуказанные микроРНК участвуют в 3 сигнальных путях: взаимосвязь с рецепторами ВКМ (33 таргетных гена), адгезивные контакты (33 таргетных гена) и сигнальный путь Hippo (69 таргетных генов) [3]. 8 миРНК с повышенной экспрессией участвуют в сигнальных путях PI3K-AKT, опухолевых клеток, клеточного цикла и регуляции актинового итоскелета [3].
Проанализировав онлайн-базу данных таргетных генов миРНК и их функциональных особенностей микроРНК (https://миРНКdb.org/; https://dianalab.e-ce.uth.gr/home), была найдена информация об экспрессии микроРНК в эндометрий и яичниках, но нет информации об экспрессии в клетках миометрия или ЛМ.
В настоящем исследовании получены данные о том, что экспрессия миРНК-20а несколько снижена в миоматозном узле в сравнении с миометрием. В литературе найдены данные об онкосупрессивных свойствах данной миРНК, а также об участии в противоопухолевых процессах в клетках меланомы [4]. Таргетные гены: HMGN2, MED12.
Авторы выявили снижение уровня экспрессии миРНК-29b в миоматозном узле в сравнении с миометрием. Множество исследований подтверждают роль миРНК в онкологических процессах. К примеру, выявлена роль миРНК-29b в холангиокарциноме [5]. Также было выявлено, что миРНК-29b имеет отрицательную обратную связь с экспрессией гена FEM1B, который ингибирует пролиферацию в опухолевых клетках легких [6]. Таргетные гены: COL5A3, COL5A1, COL3A1, DNMT3A, ADAMTS10, COL6A1.
Экспрессия миРНК-100 повышена в миометрии в сравнении с миоматозным узлом. В исследовании B. Liu и соавт. выявлено, что миРНК-100-3р стимулируют пролиферацию клеток Сертоли и синтез ДНК, подавляют апоптоз [7].
В миометрии уровень экспрессии миРНК-10b достоверно выше в сравнении с миоматозным узлом. Анализируя стволовые раковые клетки, полученные у пациентов с раком молочных желез (РМЖ), было выявлено, что миРНК-10b способствует метастазированию и миграции, а также самообновлению клеток РМЖ через сигнальные пути [8].
Экспрессия миРНК-21-5р повышена в клетках РМЖ и, по результатам ROC-анализа, может быть использована для отделения больных РМЖ от здоровых с чувствительностью 86,7% и специфичностью 93,3% [9]. Также экспрессия этого микроРНК повышена в различных онкологических клетках, таких как рак прямой кишки, глиобластома, опухоли гипофиза. В представленном исследовании экспрессия миРНК-21-5р повышена в миоматозном узле в сравнении с миометрием.
МиРНК-10a-5p регулируют ген HOXA1 в клетках рака яичников (РЯ) и тем самым подавляют агрессивные фенотипы РЯ [10]. Другие исследования показывают, как микроРНК-10а-5р регулирует гиперпролиферацию и влияет на метастазы рака шейки матки [11].
Заключение
Авторы не выявили связь между уровнем экспрессии миРНК в миоматозных узлах в зависимости от объема оперативного лечения (миомэктомия или экстирпация матки) и в группах сравнения в зависимости от рецидива заболевания в анамнезе. Напротив, выявлено снижение уровня экспрессии let7, миРНК-20a, миРНК-100, миРНК-10a в 1,5 раза в миоматозном узле по сравнению с аналогичными показателями в миометрии, а микроРНК-26а, микроРНК-29b, микроРНК-10b - в 2 раза. Что касается миРНК-21, экспрессия данной миРНК была в 1,6 раза выше в миоматозном узле по сравнению с показателем в миометрии.
Во всех проанализированных источниках (https://миРНКdb.org, https://dianalab.e-ce.uth.gr/home) не представлены данные по экспрессии миРНК в миометрии и миоматозных узлах, что делает данное исследование особенно актуальным. Выявленные различия в экспрессии 8 миРНК из 20 изученных: let7, 20a, 26a, 29b, 100, 10b, 21, 10a - доказывают, что эти нуклеотидные последовательности играют ключевую роль в формировании ЛМ и могут быть диагностическими маркерами. Это дает стимул для дальнейшего углубленного исследования в данной области.
