Clinical and morphological parallels of the consistency of the uterine scar after cesarean section
AbstractIn recent years, the circle of supporters of the management of childbirth in patients with an operated uterus through the natural birth canal has been expanding. An attempt at vaginal delivery can protect a woman from complications associated with repeated surgery, but requires a comprehensive risk assessment. The existing controversies regarding the tactics of managing patients in this cohort prove that this problem has not yet been found a clear solution and there are many reasons for this: first of all, this is the fear of uterine rupture along the existing scar. In the structure of contraindications for natural childbirth after a cesarean section (СS), the main role is given to assessing the consistency of the uterine scar.
The aim of the study was to improve the criteria for selecting women for vaginal birth after cesarean section.
Material and methods. The study involved 54 patients after repeated CS, who underwent morphological and immunohistochemical examination of excised scars. Retrospectively, based on birth histories, a comparative analysis of clinical and anamnestic factors affecting scar repair was carried out.
Results. During the study it was shown that the healing of a postoperative wound on the uterus was influenced to a greater extent by: complicated course of postoperative period (chances of scar failure were 4.48 times higher), age (3.39 times higher in women aged 35 years and older), presence of extragenital diseases (4.03 times higher), urgency (a history of emergency CS increased the likelihood of the formation of an incompetent scar by 3.53 times).

Результаты проведенного нами морфологического исследования подтвердили имеющиеся фундаментальные представления о взаимосвязи между характером развивающегося в зоне рубца репаративного процесса и интенсивностью ангиогенеза, зависящих от многих факторов. При морфологическом исследовании статистически значимым отличием явилось наличие фиброза в несостоятельном рубце на матке. В группе с несостоятельным рубцом между пучками мышечных волокон определялась плотная сформированная соединительная ткань в виде прослоек с наличием разнокалиберных сосудов: преобладали венулы, капиляры и мелкие артерии, иногда отмечались единичные артерии со склерозированной стенкой (рис. 5). Мышечные волокна, окруженные тонкими прослойками соединительной ткани, были дистрофичны. В местах, где мышечные волокна окружены плотным каркасом соединительной ткани, часто наблюдалась гипертрофия.

Из 29 несостоятельных рубцов преобладание фиброзной ткани наблюдалось: 30-40% у 9 образцов, 50% у 12 образцов, свыше 60% у 8 образцов.
Из 25 состоятельных рубцов на матке 11 были мышечного типа, 14 - смешанного типа (мышечно-рубцового, с количеством фиброзной ткани от 10 до 20%) (рис. 6, 7).

Полученные результаты согласуются с представленными работами других исследователей [18, 19]. По данным профессора К.А. Рогова и соавт., при полноценной регенерации стенки матки после КС доля соединительной ткани не должна превышать 10-15% [19].
Данные ИГХ-исследования подтвердили результаты гистологического исследования. Была выявлена статистически значимая разница в выраженности экспрессии CD31 между группами (р=0,02). В группе состоятельных рубцов выраженность экспрессии CD31 была значимо выше (58%), чем в группе с несостоятельным рубцом (28%), что свидетельствует о более выраженной васкуляризации ткани рубца в первой группе. В рубцах, представленных преимущественно мышечной тканью, отмечались также выраженная экспрессия десмина в 90% клеток и умеренная экспрессия коллагена IV типа в межклеточном пространстве (65,5%), а в группе с несостоятельным рубцом экспрессия десмина составила 24% (р=0,003).
Заключение
В результате данного исследования была выявлена зависимость тканевой полноценности миометрия от изученных факторов, что позволит конкретизировать критерии отбора женщин для вагинальных родов после перенесенного КС. Шансы несостоятельности рубца оказались в 3,39 раза выше у женщин в возрасте 35 лет и старше и при наличии ЭГЗ (в 4,03 раза). Экстренное КС в анамнезе повышало вероятность формирования несостоятельного рубца в 3,53 раза, осложненного течения послеоперационного периода - в 4,48 раза. Выявлена сопряженность гистологической картины рубца на матке с его ультразвуковой толщиной. В то же время не показали значимость такие факторы риска, как паритет, интергенетический интервал, масса тела новорожденного.
