Perimenopausal contraception: risk-reducing preferences of estetrol

Abstract

Perimenopause is a special period in a woman’s life when there is a high need for contraception, but at the same time, regular changes occur in many systems and organs associated with a growing estrogen deficiency. It is known that combined oral contraceptives remain one of the most effective methods of preventing unwanted pregnancy. But when it comes to contraception in older reproductive age, associated with an increase in a number of risks, safety and non-contraceptive effects aimed at reducing estrogen deficiency are of particular importance. Taking into account these factors, a new drug containing a unique estrogen - estetrol claims to be the optimal contraceptive.

За счет наличия в своей структуре 4 гидроксильных групп Е4 демонстрирует выраженный антиоксидантный эффект, что крайне важно для защиты нейронов от окислительного стресса [18].

Доказано благоприятное влияние Е4 на гипоксически-ишемические изменения в головном мозге: выявлено значимое снижение активности лактатдегидрогеназы - маркера клеточной деструкции зарегистрирована высокая выживаемость нейронов в присутствии Е4 в условиях гипоксического стресса, а также положительное влияние Е4 на миелинизацию нервных волокон [19].

Одно из наиболее интересных направлений в изучении свойств Е4 - его потенциальная способность снижать риски развития деменции, в том числе болезни Альцгеймера (БА). Поскольку БА развивается у женщин в 2-3 раза чаще, чем у мужчин, большинство исследователей сходятся во мнении, что снижение уровня эстрогенов в перименопаузе вносит значительный вклад в патогенез этого заболевания у женщин.

Лидирующую роль в развитии БА играют нарушения со стороны нейротрансмиттеров, особенно холинергическая дисфункция, тогда как эстрогены способны повышать уровень ацетилхолина. Еще один фактор развития деменции - сосудистый компонент, так как локальная сосудистая недостаточность отмечается у 50% больных БА. Безусловно, атеросклеротическое поражение сосудов головного мозга ухудшает течение этого заболевания, в то время как доказанное антиатеросклеротическое действие эстрогенов в сочетании с их способностью снижать риск артериальной гипертензии способно положительно повлиять на сохранение когнитивных функций [20].

Нельзя обойти стороной и эффекты комбинации Е4/ДРСП (Эстеретта®), которые обеспечивает гестагенный компонент - дроспиренон. Благодаря его выраженным антиандрогенным и антиминералокортикоидным свойствам комбинация Е4/ДРСП оказывает благоприятное действие на состояние кожи и волос, не повышает артериальное давление, а также уменьшает выраженность предменструального синдрома [21, 22].

Что не менее важно, после окончательного завершения репродуктивной функции с целью профилактики менопаузальных расстройств можно с легкостью перейти на менопаузальную гормональную терапию (МГТ). Для перехода с КОК на МГТ достаточно ОДНОГО из предлагаемых критериев:

· прекращение менструальноподобной реакции (при отсутствии беременности);

· необходимость использования барьерных методов контрацепции (следует уведомить об этом пациентку);

· климактерический синдром и генитоуринарный менопаузальный синдром на фоне применения КОК, в том числе в безгормональном периоде;

· пациенткам в возрасте 55 лет или старше, которые по какой-либо причине продолжают принимать КОК, показан перевод на непрерывную монофазную МГТ в низкой или ультранизкой дозе;

· климактерический синдром на фоне вынужденной отмены КОК;

ФСГ >30 мМЕ/л при 2-кратном измерении в безгормональный период приема КОК.

Прием МГТ можно начинать на следующий день после принятой последней в упаковке таблетки КОК [2].

Заключение

Таким образом, прием КОК в период менопаузального перехода при отсутствии факторов риска имеет значительные преимущества:

· высокая контрацептивная эффективность;

· регуляция/контроль менструального цикла;

· сохранение минеральной плотности костей и снижение риска переломов в последующем;

· профилактика гиперплазии и рака эндометрия;

· купирование вазомоторных симптомов;

· коррекция предменструального синдрома;

· профилактика рака яичников и колоректального рака.

На сегодняшний день в арсенале акушеров-гинекологов имеется инновационный контрацептив, который не только обеспечивает вышеуказанные преимущества, но и обладает оптимальным профилем безопасности с минимальным влиянием на тромбогенный и атерогенный потенциал, что особенно важно в перименопаузе. Также препарат обладает протективными свойствами в отношении молочной железы. Совокупность этих характеристик делает препарат Эстеретта® поистине уникальным, открывая для женщин старшего репродуктивного возраста новую эру в гормональной контрацепции.
