Molecular endometrial signatures associated with uterine infertility factor

Abstract

The aim of the study – to study the «receptive» and immune characteristics of the endometrium of infertile women in the “implantation window” phase.

Material and methods. 171 women were examined, with the allocation of groups with infertility: unclear genesis (n=21), with tubal-peritoneal infertility (TPI) (n=74), associated with external genital endometriosis (EGE) (n=22), with chronic endometritis (CE) (n=36), with “thin” endometrium (n=8). The control group included 10 healthy fertile women.

Serum vitamin D was evaluated, hysteroscopy and pipelle biopsy of the endometrium in the «implantation window» phase was performed to assess the pathomorphological and immunohistochemical studies of the expression of steroid hormone receptors (estradiol ER and progesterone PR), vitamin D receptors (VDR), cytokines (TNFα, IL-10). Chronic endometritis (CE) was confirmed when the CD138+ marker was detected, and the proliferative activity of the endometrium was confirmed when Ki-67 was overexpressed.

Results. After immunohistochemical examination, taking into account the hysteroscopy data and pathomorphological conclusions, the molecular signatures of the endometrium within each infertility group were heterogeneous: with impaired immune status (n=42), proliferative (n=40), dysplastic (n=47), chronic inflammation (CE) (n=30), “normal” endometrium (n=10). The “normal” endometrial variant was combined with the control group (n=22).

Significant differences in the content of 25 (OH) D in infertile women from the control group [43 (36; 52.5) were detected in the dysplastic phenotype [26.8 (24.5; 40.5); p=0.005], proliferative [23.5 (15.3; 48.0); p=0.009] and CE [28.5 (25.5; 38.8); p=0.02].

A “receptive” endometrial profile was detected in 22 women (10 fertile and 12 with a “normal” endometrial phenotype from the TPI group), a “non-receptive” profile was determined in all women with CE, 89.5% with a proliferative and dysplastic phenotype, 62.5% with a disturbed immune status. Impaired endometrial receptivity confirms a decrease in the stromal expression ratio of mean PR/ER receptor values (unlike the control group and the normal endometrial phenotype – 2.1): with proliferative – 2.6 times (p<0.05), with CE – 1.5 times (p<0.05) and dysplastic phenotype – 1.9 times (p<0.05). The level of VDR expression in the endometrium of women with all phenotypes except dysplastic was significantly higher than in the group with a “normal” endometrium in both compartments (p<0.001), with CE and impaired immune status in the epithelium of the glands – comparable, stroma – significantly lower in inflammation (p<0.001).

The molecular profile of cytokines and the immunoregulatory index of TNF-α/IL-10 at different phenotypes differed from the control (“normal” endometrium – 1.1 in the epithelium of the glands and 1:1 in the stroma): with a disturbed immune status of 1.0 and 0.94, respectively; in case of dysplastic – 1.0 and 0.7, respectively; CE – 2.5 and 2.2, respectively; proliferative – in the epithelium of the glands – 1.5.

Conclusion. The main molecular signatures of the endometrium and the determinants of its dysfunction in the uterine factor of infertility have been identified. It has been shown that the acquisition of “receptive” status by the endometrium is realized under the influence of immune responses modulated by the activity of cytokines. Impaired expression of steroid hormones (ER and PR) and indirectly affecting the receptivity of immune factors (cytokines TNF-α/IL-10, VDR) allows us to assert the revealed defects in the mechanisms of decidualization of the stroma and secretory potential of the glands, genes for controlling the processes of cellular metabolism.

The concept that favorable blastocyst implantation suggests a receptor-mediated immune dominant of a moderate pro-inflammatory decidual environment has been confirmed.

Молекулярный профиль женщин с пролиферативным фенотипом эндометрия отличался умеренной экспрессией TNFα и сниженным уровнем IL-10, кофакторность которых доказывала средняя положительная корреляционная связь (r=0,47). Возрастание экспрессии VDR подавляло передачу сигнала PR (r=-0,47).

Корреляционные связи указывали на нарушение PR-опосредованных эффектов при низком уровне IL-10 (r=-0,46) и гиперэкспрессии ER (r=-0,44) в строме. Полученные нами данные подтверждают, что нарушения взаимодействия цитокинов и их рецепторов участвуют в каскаде сложных многоэтапных механизмов и, как следствие, в развитии гиперпластических процессов эндометрия за счет регуляции процессов пролиферации, дифференцировки и апоптоза клеток [25]. Варианты морфологического субстрата (очаговая ГПЭ, ПЭ и микрополипы) обусловлены различной метаболической активностью клеток эндометрия, определяемой нарушением "кворума" локальных иммунно-гормональных ресурсов (длительность воздействия триггера и способность "поврежденной" ткани к репарации). Вероятно, развитие пролиферативных изменений у 40,0% женщин с ХЭ сопряжено с геномной нестабильностью эпителия эндометрия, зависящей от эпигенетических факторов. Возрастание пролиферативной активности (PR/ER=0,8, Ki-67) выявлено непосредственно в ткани железистых и железисто-фиброзных ПЭ, в отличие от прилежащей ткани неизмененного эндометрия.

Фенотип хронического воспаления

Особенности рецептивного профиля эндометрия при фенотипе ХЭ в фазу "имплантационного окна" заключались в некоторой гипоэкспрессии ER в железах и строме. Снижение экспрессии PR в 2 раза в эпителии желез обусловлено персистенцией ER в строме - фибропластической трансформацией. Изменения архитектоники при инфильтрации стромы плазматическими клетками с развитием дисфункции эндометрия представлены также в других источниках [8, 9]. Нарушение экспрессии рецепторов стероидных гормонов подтверждает не только нарушение децидуализации стромы, но и ее васкуляризации [26], при которых отсутствие сдвига PR из эпителия в клетки стромы препятствует подавлению экспрессии ER в эпителиоцитах желез. Отсутствие полноценного прогестерон-рецепторного взаимодействия при ХЭ, подтверждаемого не только количеством экспрессируемых рецепторов к гормонам, но и их соотношением (PR/ER=1,1), определяли независимо от морфологической картины (полноценная секреторная трансформация/несоответствие фазе менструального цикла) и уровня прогестерона в периферической крови. Данные об отсутствии корреляции содержания гормона и маркеров рецептивности эндометрия также приведены в работах других авторов [27].

Воспалительный тип иммунорегуляции при ХЭ определяла избыточность экспрессии в эндометрии TNFα при снижении IL-10 как в эпителии желез, так и в строме. Индекс TNFα/IL-10 - 2,5 (при норме 1,1 в железистых клетках) подтверждает нарушение молекулярных взаимодействий в иммунокомпетентных клетках.

Полученные нами данные позволяют утверждать, что аномальное иммунное микроокружение в период "имплантационного окна" при фенотипе ХЭ обусловлено повышенной продукцией и нарушением передачи сигналов TNFα. Избыточная индукция триггера митотической активности может быть связана с микрополипозом, уровень которого рассматривают как гистероскопический маркер ХЭ [28, 29]. Снижение экспрессии IL-10 при ХЭ предположительно является причиной количественного или функционального дефицита противовоспалительного клона Treg-клеток. Таким образом, основу патогенеза фенотипа хронического воспаления эндометрия составляет нарушение рецептивности на фоне локального иммунного дисбаланса, с превалированием провоспалительного Th1-подтипа.

При фенотипе хронического воспаления эндометрия корреляция экспрессии в эпителии желез ER и PR (r=0,6) подтверждает важную роль пролиферативной фазы менструального цикла, нарушение локального хода событий которой детерминирует морфофункциональную недостаточность секреторной фазы. Неблагоприятное влияние на имплантацию эмбриона связывают с развитием и фазовой перестройкой всех структур эндометрия в воспалительной микросреде [30].

При фенотипе ХЭ преобладает дефицит 25(ОН)D в периферической крови (86,7%), и это позволяет считать, что блокирование иммунорегуляторных эффектов витамина приводит к усилению иммунных реакций Th1-типа. Достоверность наших заключений подтверждают данные о способности при бактериальной инфекции плаценты, нейтрофилов и макрофагов, обработанных комбинацией липополисахарида и 1,25(OH)2D, к фагоцитозу [31]. Ранее были представлены данные о нарушении иммунной регуляции периода "имплантационного окна" при отклонении содержания витамина D от референтных значений: поляризация Th1-цитокинов (IFN-γ, TNFα, IL-1α и IL-1β), высокая цитотоксичность NK-клеток, снижение секреции Th2-цитокинов (IL-10) [32]. Суммация представленных в работе результатов позволяет утверждать о срыве при недостатке и дефиците витамина D в сыворотке крови компенсаторно-приспособительных механизмов - иммунной толерантности и противоинфекционной защиты.

Полученные данные отражают способность VDR-маркированных клеток эпителия желез и стромы к аберрантной экспрессии генов, кодирующих основные пути локального метаболизма витамина D. Нарушение интерфейса "VDR-25(ОН)D" меняет активность транскрипционных с усиленной поляризацией наивных T-лимфоцитов со сдвигом баланса Th1/Th2 в сторону Th1-ответа, подавлением активности Treg-клеток. Полагаем, что интегральное влияние витамина D и VDR на локальный иммунитет эндометрия должно стать предметом дальнейших исследований.

Заключение

Полученные данные позволяют утверждать о формировании фенотипически неоднородных при единой нозологической форме бесплодия (необъяснимого генеза и ассоциированным с ХЭ, ТПБ, НГЭ, "тонким" эндометрием) молекулярных сигнатур эндометрия.

Выделены молекулярно-адаптивные фенотипы эндометрия: пролиферативный, диспластический, с нарушенным иммунным статусом, хроническое воспаление (ХЭ).

Результаты наших исследований расширяют представления об имплантации как о сложной молекулярной системе коммуникаций, координируемой эндокринными, паракринными и иммунорегуляторными факторами. Выявлено, что "несинфазность" эндометрия "рецептивному" статусу при аномальных в период "окна имплантации" параметрах иммунного микроокружения сопровождается молекулярными и клеточными дефектами.
