Prediction of congenital malformations associated with diabetes mellitus

Abstract

The aim of the study was to develop a model for early diagnosis and prediction of fetal congenital malformations in pregnant women with type 1 and 2 diabetes mellitus.

Material and methods. Retrospective analysis of the history of pregnancy and parameters of first ultrasound study (at 6.0–9.0 weeks) in pregnant women with and without fetal abnormalities in diabetes mellitus.

Results. Statistically significant differences between the groups were found in: fasting venous plasma glucose, glycated hemoglobin, pregravid preparation, deviations in the mean-internal diameter of the fetal bladder and the diameter of the yolk sac on the first ultrasound examination at 6.0–9.0 weeks of gestation (р<0.001). Using the discriminant analysis method, we developed a mathematical model for early (before the first screening) prediction of the probability of developing the risk of fetal abnormalities in pregnant women with diabetes mellitus with a sensitivity and specificity of 90%.

Conclusion. On the basis of the performed study, the data obtained can be used for early prediction of fetal abnormalities in pregnant women with diabetes.

В результате дискриминантного анализа зависимости вероятности развития ВПР плода у беременных с СД1 и СД2 от количественных лабораторных данных и результатов 1-го УЗИ была получена следующая модель раннего (6,0-9,0 нед гестации) прогнозирования ВПР плода (1):

YВПР = -4,5+0,4×ХГЛ+0,7×ХГЛИК-0,9×ХСВДПЯ-0,2×ХСВДЖМ, (1)

где: YВПР - дискриминантная функция, характеризующая вероятность развития ВПР плода у беременных с СД; -4,5 - константа дискриминантной функции; ХГЛ - глюкоза венозной плазмы натощак (ммоль/л) в I триместре беременности при постановке на учет в ЖК; ХГЛИК - уровень гликированного гемоглобина в I триместре беременности при постановке на учет в ЖК; ХСВДПЯ - средний внутренний диаметр плодного яйца на УЗИ в 6,0-9,0 нед гестации (1 - <10П, 2 - 10-90П, 3 - >90П для данного гестационного срока); ХСВДЖМ - средний внутренний диаметр ЖМ (мм).

Константа дискриминации составила -0,31, p=0,00001. Исходя из коэффициентов в уравнении (1), вероятность формирования ВПР у беременных с СД1, СД2 возрастала при увеличении глюкозы венозной плазмы натощак и повышении уровня гликированного гемоглобина в I триместре беременности, а также при снижении на УЗИ СВДПЯ и СВДЖМ.

Вероятность принадлежности беременных с СД к группе высокого или низкого риска по развитию ВПР плода определялась исходя из рассчитанных значений прогностической дискриминантной функции (1): при значении функции более -0,31 пациентку относили к группе высокого риска развития ВПР, при значении функции менее -0,31 - к группе низкого риска. Чувствительность модели составила 90,0%, специфичность - 90,0%.

Обсуждение

Целью нашего исследования была оценка анамнестических, лабораторных и УЗ-параметров для разработки модели раннего прогнозирования ВПР плода у беременных с СД. Мы обнаружили, что предположить неблагоприятные исходы (ВПР) можно не только по анамнестическим факторам, увеличению целевых значений глюкозы и гликированного гемоглобина, но также и по изменению СВДПЯ и СВДЖМ во время 1-го УЗИ (в 6,0-9,0 нед гестации). Отклонения в лабораторных и УЗ-параметрах достоверно чаще встречались

у беременных основной группы. Медианы глюкозы и гликированного гемоглобина, взятые при постановке беременной на учет в ЖК, были значимо выше у пациенток основной группы - это, несомненно, подтверждает данные международных и отечественных исследований, что основной причиной эмбриофетопатии является антенатальное воздействие гипергликемии [1, 2, 5, 7, 8].

Отмечено, что в контрольной группе женщины достоверно чаще соблюдали ПГП. Исходя из полученных данных, ПГП снижает риск развития ВПР в 8,3 раза, что подтверждает мнение многих авторов о том, что вероятность развития ВПР связана с длительностью СД до беременности и уровнем гликемии во время гисто- и органогенеза, а риск возникновения аномалий развития может быть уменьшен при строгом гликемическом контроле в период ПГП [4, 6, 10].

Медиана размера СВДЖМ была значительно ниже (2,5 против 4,0 мм) в основной группе, что подтверждает одну из основных теорий механизма развития ВПР - повреждение ткани ЖМ вследствие гипоксии, обусловленной гипергликемией [9, 11].

Заключение

Результаты проведенного исследования позволили разработать метод раннего (до 1-го скрининга) прогнозирования вероятности развития ВПР плода у беременных с СД1, СД2 с чувствительностью и специфичностью 90%.
