Gestational diabetes mellitus associated with subclinical hypothyroidism. Maternal and perinatal outcomes
AbstractThe aim of the study was to evaluate maternal and perinatal complications in patients with gestational diabetes mellitus (GDM) associated with subclinical hypothyroidism (SHT).
Material and methods. A retrospective cohort study included 338 patients: 53 with GDM, 160 with SHT, 125 with GDM+SHT, who were observed and delivered at the Perinatal Center of the City Clinical Hospital # 29 named after N.E. Bauman in 2022. The data were obtained by analyzing the birth histories, including anamnesis, term and method of delivery of patients, their body mass index, the presence of the following pregnancy complications: anemia, preeclampsia (PE); in newborns – birth weight, Apgar score, the need for resuscitation, the presence of signs of diabetic fetopathy (DF).
Results. All three groups were comparable in age. In all groups, there was a high incidence of impaired fat metabolism (85; 68.3; 67.2%), with a significant predominance in the group of patients with GDM [p(GDM – GDM+SHT)=0.0171; p(GDM–SHT)=0,216]. The following complications of pregnancy and childbirth were identified by groups: GDM, SHT and GDM+SHT, respectively: PE (7.5; 25; 10.5%), anemia (32.1; 50.6; 61.3%), macrosomia (24.5; 7.5; 17.7%), DF (9.3; 0; 15.5%), abdominal delivery (13.2; 20; 37.1%). The body weight of newborns was higher in the GDM+SHT group compared to the SHT group (3676.72±268.71 p=0.0062). The combination of GDM and SHT significantly increased the chances of anemia (RR=1.926, 95% CI 1.271–2.919; OR=3.285, 95% CI 1.665–6.48). The presence of SHT in patients with GDM increased the frequency of operative delivery (RR=4.876, 95% CI 1.849–12.861; OR=7.133, 95% CI 2.417–21.048). It is noteworthy that abdominal delivery was performed more often in the group of patients with SHT than in the GDM group (13.2 vs 20%). The average values of the newborn’s Apgar score at 1 and 5 minutes were significantly lower in the SHT group compared to the GDM group [(p(1’)=0.0006; p(5’)=0.0001] and also significantly lower in the GDM+SHT group compared to the SHT group [p(1’)= 0.0022; p(5’)=0.0037].
Conclusion. The results obtained confirmed the implementation of BMI in the development of endocrine diseases during pregnancy. The degree of obesity was more pronounced in patients with the presence of SHT. The combination of GDM and SHT worsened the course of the gestational process and determined the method of delivery: anemic syndrome was more often detected, the frequency of CS increased. The comorbidity of SHT and GDM did not affect the frequency of macrosomia, DF and the need for intensive neonatal care, however, it reduced the newborns Apgar score. Considering TSH not only as an indicator of SHT, but also as a predictor of GDM, timely prevention and diagnosis of GDM becomes possible. Further studies are needed to assess maternal and perinatal outcomes, depending on the timing of diagnosis and the degree of SHT compensation.

Получена достоверная разница в количестве случаев анемии: женщины с ГСД, ассоциированным с СГТ (см. табл. 1), чаще страдали анемическим синдромом по сравнению с группой ГСД вне зависимости от вида терапии ГСД (ДТ, p=0,0036, ИТ, p=0,0154). Таким образом, СГТ является достоверно значимым фактором риска развития анемии у беременных (ОШ=3,285, 95% ДИ 1,665-6,48), что согласуется с данными литературы. Y. Yang и соавт. (2020) в своем исследовании также пришли к выводу, что гестационный гипотиреоз повышает риск развития анемии. Авторы отметили, что при гипотиреозе наблюдались значительное снижение гемоглобина и повышенный риск анемии во второй половине беременности [16].
Полученные показатели частоты ПЭ также подтвердили данные литературы о значении СГТ в развитии ПЭ у пациенток с ГСД (ОР=1,738,95% ДИ 0,471-4,032; ОШ=1,422, 95% ДИ 0,441-4,581] [17]. Однако статистической достоверности нами выявлено не было.
На рис. 3 представлена частота материнских и перинатальных осложнений в исследуемых группах.

При анализе частоты абдоминального родоразрешения установлено, что и СГТ (ОР=4,876, 95% ДИ 1,849-12,861; ОШ=7,133, 95% ДИ 2,417-21,048) и ГСД (ОР=1,84, 95% ДИ 1,251-2,706; ОШ=2,329, 95% ДИ 1,369-3,962) являлись значимыми факторами риска кесарева сечения (КС) в группе эндокринной коморбидности. Это согласуется с данными W.Y. Liu и соавт. (2022), свидетельствующими о том, что частота КС была выше у женщин с гипотиреозом, чем у эутироидных женщин [18].
Установлено, что пациентки с ГСД, ассоциированным с СГТ, родоразрешались раньше, что можно объяснить показателями со стороны матери и плода (ПЭ и ДФ) (табл. 2)

Анализ массы тела новорожденных показал, что новорожденные в группе СГТ были меньше по массе, чем в коморбидной группе (3594,75±232,45 и 3651,82±267,64 г соответственно, p=0,0062) (рис. 4), что согласуется с частотой диагностирования макросомии, которая чаще наблюдалась при ГСД+СГТ (p=0,0099). Что касается ДФ, то она встречалась чаще в коморбидной группе (15,5 против 9,3%), однако, по нашим данным, не было выявлено статистической достоверности полученных результатов.
Несмотря на то что у новорожденных во всех группах не было асфиксии в родах, имелись различия в оценке по шкале Апгар: наиболее низкая - в группе сочетания ГСД и СГТ (см. табл. 2). Наличие у пациенток СГТ достоверно снижало оценку по шкале Апгар, что согласуется с данными K. Mahadik и соавт. (2020), в работе которых была доказана связь низкого балла по Апгар - 21,1% (p=0,042) с наличием у матери гипотиреоза [19]. Однако, что касается частоты поступления новорожденных в отделении реанимации и интенсивной терапии (ОРИТН), полученные данные расходятся с результатами исследования G. Ducarme и соавт. (2021), которые в своей работе пришли к выводу о том, что у женщин с СГТ и ГСД частота поступления новорожденных в ОРИТН в течение 24 ч после рождения была значительно выше, чем у женщин с изолированным ГСД (33,3 против 5,7%) [20]. Так, настоящее исследование показало, что новорожденные от матерей с ГСД чаще нуждались в интенсивном наблюдении (ОР=1,621, 95% ДИ 1,047-2,512; ОШ=2,063, 95% ДИ 1,047-4,064). Во многом это объясняется характерными для таких новорожденных проявлениями респираторного дистресс-синдрома, гипогликемии и нарушения адаптации [13]. При пересчете с поправкой на вид терапии ГСД оказалось, что дети от матерей с ГСД на ДТ чаще нуждались в ОРИТ (p=0,0455), что можно объяснить недостаточным контролем гликемии в ходе гестационного процесса и неэффективностью ДТ.

Заключение
Во всех группах установлена высокая частота ожирения, наиболее высокая у пациенток с ГСД, что подтверждает значимость ИМТ в развитии эндокринных заболеваний при беременности. Однако степень ожирения была более выражена у пациенток с наличием СГТ. Сочетание ГСД и СГТ у пациенток ухудшало течение гестационного процесса и влияло на метод родоразрешения: чаще выявлялся анемический синдром, возрастала частота абдоминального родоразрешения. Коморбидность ГСД и СГТ не влияла на частоту макросомии, ДФ, снижала оценку новорожденных по шкале Апгар, но при этом не требовала интенсивного неонатального ухода. Учитывая тот факт, что в данное исследование вошли пациентки с поздней диагностикой СГТ (после 12 нед гестации), необходимы дальнейшее изучение материнских и перинатальных исходов в зависимости от сроков диагностики и компенсации СГТ, а также углубленное изучение механизмов метаболических нарушений. Если рассматривать ТТГ не только как показатель СТГ, но и как предиктор ГСД, становится возможна реализация своевременной профилактики и диагностики ГСД.
