Sexual dysfunction in female healthcare workers in late reproductive age and perimenopause

Abstract

The aim of the study was to determine the frequency and structure of sexual dysfunctions in women of late reproductive age and in the period of perimenopause.

Material and methods. The sexual function of 121 relatively healthy women was studied using a clinical interview and the Female Sexual Function Index (FSFI) questionnaire.

Results. Sexual dysfunction was found in 35.5% of the respondents. Women with sexual dysfunction are more likely to have benign breast dysplasia. The most pronounced disorders were observed in the mental component of the sexual sphere both in the study group and in the control group. In women of late reproductive age and in the perimenopausal period, a decrease in motivation for sexual activity was most often detected.

Conclusion. Interpretation of the FSFI questionnaire requires adjustment for middle-aged women.

Проведенное исследование позволило выявить структурные различия сексуального функционирования женщин позднего репродуктивного и перименопаузального периодов. Полученные результаты демонстрируют важность психического компонента в расстройстве сексуальной функции у женщин в указанный период жизни (напомним, что на момент исследования участницы анкетирования не имели жалоб относительно гинекологических заболеваний). Низкий показатель домена "влечение" в группе контроля может говорить о том, что для женщин данной категории целесообразно создать корректирующий коэффициент, выравнивающий общий балл, или выработать другой, более низкий показатель нормы. Самый высокий индекс сексуальной функции в группе контроля (2-я группа) составил 32,7 при максимальном значении 36, а средний индекс 29,78 приближается к границе 26,55, разделяющей доклинические и клинические нарушения. Данные других исследователей показывают схожие результаты при исследовании групп женщин того же возраста [19-21]. Целесообразно учитывать социокультурные особенности данной группы исследуемых, не позволяющие всегда искренне отвечать на вопросы ИЖСФ. Для решения этих вопросов необходимы дальнейшие исследования с включением относительно здоровых женщин среднего возраста.

Следует отметить, что нами также была предпринята попытка анкетирования персонала медицинской организации, не имеющего медицинского образования. Однако в большинстве случаев предложение пройти опрос воспринималось крайне негативно, поэтому мы были ограничены сведениями, полученными от женщин-медиков.

Заключение

В ходе исследования выявлено, что наиболее уязвимым фактором нарушения сексуальной функции женщин была его психологическая составляющая. Личный анамнез в отношении данных о заболеваемости COVID-19 представляет интерес в плане оценки состояния сексуальной функции. При сравнении показателей женской сексуальной функции по доменам опросника ИЖСФ получены статистически значимые различия в структуре нарушений, более значимые из которых относились к психической сфере. Учитывая особенности женщин среднего возраста, целесообразно разработать для них модифицированный метод оценки ИЖСФ.
