A clinical case of acute progression of feto-fetal transfusion syndrome

Abstract

Background. Pregnancy with monochorionic twins continues to be one of the most pressing problems in obstetrics due to the high risk of specific pregnancy complications associated with the monochorionic type of placentation and non-specific complications (premature birth, preeclampsia) associated with multiple births. Feto-fetal transfusion syndrome (FFTS) is one of the specific complications of pregnancy, leading to high perinatal morbidity and mortality. The existing Quintero classification of the stages of FFTS reflects the linear sequential progression of the stages of the syndrome without indicating a possible atypical or rapid progression of FFTS.

Material and methods. Our article presents a clinical observation of a pregnant patient with monochorionic twins, whose pregnancy was complicated by rapid development of FFTS. We described in detail dynamics of ultrasound examinations, tactics of management of this patient.

Conclusion. Given the possibility of rapid and atypical course of FFTS, ultrasound monitoring (intervals between studies) in monochorionic type of placentation remains one of the most pressing issues in the management of such patients. The appearance of even a slight discordance in the amount of amniotic fluid that does not meet the criteria of stage 1 of FFTS requires a change in the existing management algorithm and ultrasound monitoring after 1–3 days from the appearance of discordance in the amount of amniotic fluid.

Используемая классификация стадий СФФТ по Quintero подразумевает линейное последовательное прогрессирование СФФТ от стадии к стадии. Однако наш опыт и данные литературы показывают, что последовательность стадий соблюдается не всегда. Так, например, I стадия сразу может перейти в V, минуя II, III, IV стадии, либо может иметь место так называемый атипичный СФФТ, при котором дискордантность по количеству околоплодных вод не отвечает критериям I стадии по Quintero, но сопровождается нарушениями допплерометрии или водянкой плода, характерными для III и IV стадий по Quintero [26].

Представляем клиническое наблюдение, демонстрирующее стремительное прогрессирование СФФТ.

Пациентка К., 38 лет, обратилась в клинический госпиталь MD GROUP ГК "Мать и Дитя" при сроке беременности 12 нед. Диагностирована монохориальная биамниотическая двойня. Беременность первая, наступила самопроизвольно. Согласно рекомендациям ISUOG, начиная с 16 нед гестации ультразвуковой мониторинг проводился каждые 2 нед с обязательной оценкой количества околоплодных вод у обоих плодов (МВК), показателей фетометрии, состояния шейки матки, наличия или отсутствия эхо-тени мочевого пузыря, определения пульсационного индекса (PI) в артерии пуповины с 16 нед, максимальной скорости кровотока средней мозговой артерии (Vmax СМА) с 20 нед беременности.

До срока 23 нед 5 дней беременности дискордантности по количеству околоплодых вод, размерам плода и нарушений показателей допплерометрии у пациентки не выявлено. При УЗИ в 23 нед 5 дней впервые обнаружена разница в объеме околоплодной жидкости у плодов: МВК 1-го плода составил 28 мм, 2-го плода - 60 мм, эхо-тень мочевого пузыря у обоих плодов визуализировалась, показатели допплерометрии оставались в норме. Таким образом, оснований для постановки диагноза СФФТ на момент проведения исследования не было.

Следующее УЗИ было проведено через 1 нед (24 нед и 6 дней). Диагностирован СФФТ, III стадия по Quintero: МВК 1-го плода (донор) - 10 мм, 2-го плода (реципиент) - 105 мм, эхо-тень мочевого пузыря 1-го плода не визуализировалась, Vmax СМА 1-го плода - 30 см/с, Vmax СМА 2-го плода - 22 см/с, показатель PI артерии пуповины 1-го плода (донора) в пределах нормы, 2-го плода (реципиента) - нулевой конечно-диастолический кровоток. В венозном протоке 2-го плода (реципиента) фиксируется нулевой конечно-диастолический компонент, кровоток в легочной артерии 2-го плода (реципиент) двунаправленный, отмечается трикуспидальная регургитация, плацента расположена по передней стенке.

На следующий день под спинальной анестезией с помощью оборудования Karl Shtorz и диодного лазера Dornier MedTech Medilas MultiBeam (длина волны - 940 нм, мощность - 33 Вт) пациентке были выполнены ФЛКА и амниодренирование. Во время операции проведена коагуляция двух АВ-анастомозов среднего калибра и 5 АВ-анастомозов мелкого калибра, АА-анастомозы не выявлены. Выполнено амниодренирование в количестве 1300 мл.

Через 1 сут после операции отмечена положительная динамика: показатели допплерометрии в венозном протоке и в артерии пуповины 2-го плода - в пределах нормы, МВК 2-го плода - 73 мм, 1-го плода - 18 мм, мочевой пузырь визуализировался у обоих плодов.

При дальнейших УЗИ количество околоплодных вод у плодов соответствовало сроку, однако обращала на себя внимание дискордантность по Vmax в СМА у обоих плодов (21 см/с - <1 МоМ и 51-55 см/с - >1,5 МоМ), что вызывало подозрения в отношении развития посткоагуляционного САП и требовало динамического ультразвукового контроля каждые 5-7 дней. Но при последующих исследованиях отрицательной динамики по Vmax в СМА обоих плодов не отмечено.

В 30-31-ю неделю беременности на фоне подъема температуры до 37,5 °С вследствие новой коронавирусной инфекции (COVID-19) произошел ПРПО, развилась родовая деятельность, в связи с чем пациентка была экстренно родоразрешена путем кесарева сечения.

Первой извлечена живая недоношенная девочка с массой тела 1529 г, оценкой по шкале Апгар 7-8 баллов, по шкале Сильвермана 6 баллов, гемоглобин при рождении - 90 г/л, что потребовало переливания эритроцитарной взвеси из расчета 20 мл/кг с положительным эффектом (гемоглобин после гемотрансфузии - 142 г/л), ретикулоциты при рождении - 81. Учитывая реализацию синдрома дыхательных расстройств (СДР), эндотрахеально введен Куросурф из расчета 200 мг/кг. Проводилась респираторная поддержка с помощью искусственной вентиляции легких (ИВЛ), новорожденная экстубирована через 1 сут.

Второй извлечена живая недоношенная девочка с массой тела 1519 г, оценкой по шкале Апгар 7-8 баллов, по шкале Сильвермана 6 баллов. Гемоглобин при рождении - 202 г/л, ретикулоциты - 78. Учитывая наличие СДР, дважды вводился Куросурф из расчета 200 и 160 мг/кг. Проводилась респираторная поддержка с помощью ИВЛ, девочка экстубирована на 3-и сутки.

Следует отметить, что ПЦР-тесты на COVID-19 у обоих новорожденных были отрицательные. На 9-е сутки дети переведены на 2-й этап выхаживания и выписаны из госпиталя в возрасте 1 мес. В настоящее время развитие детей соответствует возрастной норме.

В данном клиническом наблюдении мы столкнулись со стремительным прогрессированием СФФТ до III стадии и постепенным медленным развитием посткоагуляционного САП за счет функционирования остаточных мельчайших АВ-анастомозов. Однако за время мониторинга в антенатальном периоде Vmax CМА донора не превышала 55 см/с, других показателей, свидетельствующих в пользу подозрения на страдание плодов не фиксировано, показаний для внутриутробного переливания крови или родоразрешения также не выявлено.

Постнатально констатировано, что дискордантность новорожденных по уровню гемоглобина составила 112 г/л (диагностический критерий для САП по разнице уровней гемоглобина ≥80 г/л), однако соотношение уровней ретикулоцитов обоих плодов былоо 1,03 (диагностический критерий для САП - >1,7). Мы считаем, что такая значительная разница в уровне гемоглобина у плодов больше обусловлена началом родовой деятельности, нежели прогрессированием САП.

Заключение

Таким образом, беременные двойней с монохориальным типом плацентации относятся к группе высокого риска акушерских и перинатальных осложнений. Несмотря на четко разработанный алгоритм ультразвукового сопровождения, течение таких беременностей остается одной из самых сложных задач в акушерстве, так как угрожает внезапным и непредсказуемым развитием СФФТ даже в поздние для этого осложнения сроки гестации (25-26 нед).
