Obesity is a risk factor of recurrent miscarriage
AbstractDespite numerous scientific studies on the analysis of the causes of recurrent miscarriage, up to 40% of risk factors remain uncertain/unknown. This circumstance determined the direction of our study.
The aim of the study was to analyze and summarize the scientific data accumulated to date on the pathogenesis of recurrent miscarriage in obese women.
Material and methods. A search was made for scientific publications for 2012–2022 in the Cochrane Library, Science Direct, PubMed, ELibrary databases using the keywords: pathogenesis, miscarriage, obesity, risk factor. Obesity is a key trigger in the pathogenesis of recurrent miscarriage in women of fertile age.
Conclusion. To date, the pathogenesis has not been fully disclosed, which makes it difficult to predict the outcomes of the above disorders. There is an obvious need for well-designed, high-quality studies whose results will allow development of effective patient management algorithms and improve reproductive/perinatal outcomes.

Доказана роль гормона роста в успешной репродукции, его влияние на овариальный стероидогенез и рецептивность эндометрия. Кроме того, гормон роста усиливает рост примордиальных фолликулов и предотвращает их атрезию, взаимодействует с гонадотропинами на более поздних стадиях фолликулогенеза и лютеинизации, а также способствует отбору и развитию доминантного фолликула [21].
У пациенток, страдающих ожирением, отмечено снижение уровня гормона роста в сыворотке крови и повышение скорости его выведения [25, 26]. Этому способствуют многие факторы, в частности нарушение регуляции соматолиберина, соматостатина, а также гиперинсулинемия и избыток циркулирующих свободных жирных кислот [21].
Учитывая признанное негативное влияние ожирения на эмбрион, эндометрий и яйцеклетки, разумно предположить, что частота выкидышей будет выше в популяции с ожирением.
Установлена связь между избыточной массой тела и высокой вероятностью потери беременности. Анализ исходов 18 481 нерожавших женщин показал, что по сравнению с данными нормовесных женщин ожирение ассоциировано с повышением риска ранней потери плода [относительный риск (ОР) 1,51; 95% доверительный интервал (ДИ) 1,15-1,99]. Дальнейший анализ показал, что ожирение достоверно связано с высокой частотой спонтанных абортов (ОР 1,51; 95% ДИ 1,13-2,02) [27], а вероятность потери беременности в 1,45 раза выше у женщин с ожирением, чем у нормовесных (ОР 1,45; 95% ДИ 1,06-1,98, p=0,021) [28].
Таким образом, ожирение, являющееся мировой проблемой XXI в., несомненно, оказывает как системное воздействие на гипоталамо-гипофизарно-яичниковую связь, вызывая эндокринопатию, так и местное, приводящее к нарушению фолликулогенеза яичников и вызывающее эндометриопатию, в свою очередь приводящие к потерям беременности. Вышесказанное свидетельствует о необходимости мультидисциплинарного подхода к профилактике и лечению ПНБ у женщин с ожирением с участием врачей различного профиля: эндокринологов, диетологов, акушеров-гинекологов.
Заключение
Ожирение является одним из факторов риска невынашивания беременности. Очевидно негативное влияние ожирения на качество яйцеклеток, эмбрионов как непосредственно, так и через механизмы эндометриопатии. Необходимы дальнейшие исследования, которые позволят точнее оценить вклад различных факторов риска и патогенеза в формирование ПНБ.
