Immunological aspects of implantation disorders in uterine infertility

Abstract

The aim of the research was to study characteristics of endometrium of infertile women in the «implantation window» phase (morphological, immunohistochemical and types of microbiota).

Material and methods. 84 infertile women were examined, with the allocation of groups: the 1st – with tubal-peritoneal infertility (TPI) (n=44), the 2nd – with chronic endometritis (CE) (n=28), the 3rd – with external genital endometriosis (EGE) (n=12).

The comparison group included 10 healthy fertile women.

Hysteroscopy and pipelle biopsy of the endometrium in the «implantation window» phase were performed to assess microbiota by real-time polymerase chain reaction (Femoflor 16 tests). Pathomorphological and immunohistochemical studies of the expression of cytokines, chemokines, growth factors (TNFα, IL-10, NRF2, GM-CSF, CXCL16, ВСА1, TGF-β) were also performed. Chronic endometritis (CE) was confirmed when the CD138+ marker was detected, and the proliferative activity of the endometrium was confirmed when Ki-67 was overexpressed.

Results. According to the results of morphological examination of endometrial biopsies of all women (with CE, TPI, EGE), histotypes were identified – proliferative [endometrial polyp (EP) (35.0%), focal simple endometrial hyperplasia (EH) (27.5%) and micropolypes (32.5%)]; CE (16.7%, in combination with micropolypes – 50.0%, endometrial polyp (EP) – 13.3%, EH – 20.0%; «normal» (in the group with TPI, n=12).

The basis for determining endometrial phenotypes was the data of immunohistochemical studies of marker expression in the «implantation window» phase. In the group with CE, the following were distinguished: the picture of true inflammation (n=12), a proliferative type (n=8), a combination of inflammation with EP (n=8). The endometrium of women with EGE was represented by a proliferative phenotype (n=12), with TPI – proliferative (n=12), CE (n=20), a variant of «normal» phenotype (n=12). Women with «normal» endometrial phenotype were distinguished by the balanced secretion of cytokines with moderate dominance of inflammatory molecular network and lactobacillar type.

The inflammatory type of immunoregulation in CE was determined by synchronous trends in predominance of proinflammatory cytokines in the gland epithelium and stroma in comparison with anti-inflammatory ones (increased TNFα, GM-CSF, CXCL16, BCA1, decreased IL-10), with the lowest TGF-β index (1 point). The endometrial microbiota of women with CE was represented by non-lactobacillar type (63.3%) – twice as often as with the proliferative type (p=0.01, χ2=6.8) due to dysbiotic profile (47.6 vs 5.3%, p<0.001, χ2=15.9).

Risk of implantation disorders in case of proliferative phenotype is due to the immunomodulatory activity of GM-CSF (1.2 times higher in glandular cells than in the control, 1.2 times lower in stroma) on the background of moderate overexpression of TNFα, CXCL16 only in epithelial glands and a decrease in IL-10 – in stroma significantly more than in glands in comparison with control. Violations of uterine microbiome in the group were less common than in case of CE (non-lactobacillar profile – 31.6%, mixed – 26.3%).

The inflammatory basis of EP and focal endometrial hyperplasia in 28.6 and 28.6%, respectively, is proved by positive expression of CD138+ and excessive expression of Ki-67.

Conclusion. The introduction of the concept of a proper immune microenvironment of the endometrium into the phase of «implantation window» allows us to distinguish types of immune inflammation that differ from the «normal» phenotype: excessive (СE phenotype) and autoimmune inflammation (proliferative phenotype). The nature of molecular-adaptive interaction of microbiota and immunocompetent cells serves as a criterion for readiness of endometrium for blastocyst implantation.

Детализация состава микробиоты смешанного типа при различных фенотипах эндометрия представлена на рис. 14.

Достоверных различий видового и количественного состава микробиоты матки в группах не выявлено, однако микроорганизмы Gardnerella vaginalis и Streptococcus spp. обнаружены только при пролиферативном фенотипе.

Особенности дисбиотичного профиля микробиоты эндометрия представлены на рис. 15.

Основу нелактобациллярного типа микробиоты эндометрия при различных фенотипах составляли вагиноз-ассоциированные инфекты с вариабельностью бактериальной ДНК в диапазоне 103-107 ГЭ/мл. Микроорганизмы Gardnerella vaginalis, Ureaplasma spp. и миксты Atopobium vaginae/Enterobacteriaceae в высоких титрах (105-107 ГЭ/мл) определяли только при ХЭ. По частоте и степени обсемененности другими представителями условно-патогенной флоры достоверных различий не выявлено (103 КОЕ/мл). Облигатных патогенов в эндометрии всех обследованных женщин не выявлено.

Обсуждение

Женщин с нормальным фенотипом эндометрия отличала сбалансированность секреции цитокинов при умеренной активации воспалительной молекулярной сети, модулируемой клетками стромы для контроля имплантации, миграции и инвазии трофобласта. Провоспалительный Th1-иммунный ответ рассматривают как необходимую для имплантации бластоцисты активность биологически активных веществ, сочетанную с децидуальной модификацией структур эндометрия [18]. Ведущую роль в управлении сетью иммунорегуляторных молекул связывают с гиперэкспрессией TNFα, высвобождаемого маточными макрофагами, развитием не только местного воспалительного ответа, но и индукцией толерогенных свойств дендритных клеток [19].

Полагаем, что преобладание в эндометрии лактобациллярного типа микробиоты следует рассматривать с позиций участия в ограничении воспалительного иммунного ответа и активации механизма иммунологической толерантности в присутствии Treg-доминанты в соотношении Treg/Th17 [15]. Именно эти иммуномикробные взаимодействия выступают критерием вероятности имплантации бластоцисты. Очевидно, микробный гомеостаз в эндометрии формирует не только устойчивость к колонизации условно-патогенной флорой, но и способность к экспрессии генов, влияющих на адекватный уровень рецепторов к стероидным гормонам в фазу "имплантационного окна" [20].

Воспалительный тип иммунорегуляции при ХЭ определяла избыточность экспрессии в железистом эпителии эндометрия провоспалительных цитокинов в сравнении с противовоспалительными (повышение TNFα - в 1,1 раза, GM-CSF, СXCL16, BCA1 - в 1,2 раза, снижение IL-10 - в 2 раза). В клетках стромы эндометрия уровень экспрессии GM-CSF, TNFα, СXCL16 оказался достоверно выше, чем в контроле, - в 1,2 раза, IL-10 - в 1,8 раза ниже, уровень TGF-β был наименьшим в сравнении с прочими группами (1 балл).

Показатель иммунорегуляторного индекса TNFα/IL-10 в железистых клетках, равный 2,5 (при норме 1,1 в железистых клетках), позволяет утверждать, что основу патогенеза фенотипа хронического воспаления эндометрия составляют нарушения молекулярных взаимодействий иммунокомпетентных клеток.

В группе с нормальным фенотипом эндометрия умеренное возрастание TNFα необходимо для правильной дифференцировки и развития клеток трофобласта, при ХЭ гиперэкспрессия маркера сопряжена с риском неудач имплантации [21].

Аномальное иммунное микроокружение воспаленного эндометрия связывают с внедрением патогенных бактерий, основной компонент внешней мембраны которых (липополисахарид) опосредует повышение уровня хемокина CXCL13 (BCA1), воспаление и рекрутирование в строму пула В-клеток [22]. Инфильтрация стромы плазматическими клетками приводит к изменениям архитектоники и дисфункции эндометрия. Полагают, что снижение экспрессии TGF-β и IL-10 при ХЭ является причиной количественного или функционального дефицита противовоспалительного клона Treg-клеток на фоне преобладания воспалительной реакции стромы, фиброза и неудач имплантации [23].

Дисбиотический профиль эндометрия у 47,6% бесплодных женщин с ХЭ представляется причиной неблагоприятных для имплантации молекулярных механизмов [10]. Нарушение микробного гомеостаза при ХЭ (преобладание нелактобациллярного профиля в 63,3%, как и чрезмерный рост Gardnerella vaginalis, Ureaplasma spp. и микстов Atopobium vaginae/Enterobacteriaceae в отсутствие пула лактобацилл), вероятно, стимулирует избыточную экспрессию провоспалительных цитокинов, создающих агрессивную для имплантации бластоцисты микросреду. Доминанту нелактобациллярного типа микробиоты эндометрия связывают с нарушением децидуализации за счет активации Toll-подобных рецепторов в железистом эпителии и в стромальных клетках [24].

Полагаем, выявленная нами гетерогенность состава микробиома матки у женщин с ХЭ (лактобациллярный, смешанный, дисбиотический) объясняет неоднородность приводимых данных о репродуктивных исходах - от бесплодия, рецидивирующих неудач имплантации и выкидышей до успешного родоразрешения. Факт наличия гетерогенности состава микробиома матки у женщин с ХЭ доказывает участие в регуляции сложных механизмов имплантации не только микробиома матки, но и локальной иммунной системы, синергичной с активностью рецепторного аппарата эндометрия [25, 26]. Полученные данные отражают идею концепции развития ХЭ вследствие нарушенного воспалительного состояния эндометрия, причинами которого полагают гормональные, инфекционные, фиброзные и аутоиммунные нарушения [27].

Вероятно, развитие определенного фенотипа эндометрия обусловлено не наличием микробиоты, особенно ее условно-патогенных представителей, а реакциями иммунной системы, индуцируемыми микробными лигандами при связывании с рецепторами эндометрия и характером экспрессии провоспалительных факторов, хемокинов и антимикробных метаболитов [28].

Наши данные дополняют представления о срыве при ХЭ механизмов адаптации к постоянному воздействию микробов и, как следствие, неадекватности иммунного ответа с развитием аутоиммунных реакций и порочного круга хронического воспаления [29].

Молекулярный профиль женщин с пролиферативным фенотипом эндометрия отличался повышенной экспрессией воспалительных цитокинов: GM-CSF - в 1,2 раза, умеренной - TNFα, СXCL16 - в 1,1 раза; сниженным уровнем IL-10 - в 1,3 раза. В клетках стромы эндометрия значимые отличия от контроля были выявлены только в отношении GM-CSF - в 1,2 раза, IL-10 - в 1,6 раза и ТGF-β - в 1,5 раза за счет вариативности диапазона значений (при сопоставимых средних значениях Me). Соотношение TNFα/IL-10 составило 1,6.

Дисбаланс локальной продукции цитокинов подтверждает участие в формировании пролиферативного фенотипа эндометрия не только провоспалительных цитокинов (GM-CSF, TNFα, СXCL16), избыточную экспрессию которых связывают с возрастанием пула цитотоксических Т-клеток, но и с нарушением локального стероидогенеза.

При пролиферативном фенотипе эндометрия нарушения микробиома матки встречались реже, чем в группе с ХЭ (нелактобациллярный профиль - 31,6%, смешанный - 26,3%). Именно снижение лактобактерий при относительном увеличении числа микроорганизмов Firmicutes, Proteobacteria, Actinobacteria, Fusobacteria, Bacteroides, E. coli и Bacteroides fragilis отмечено при ГПЭ [30]. Низкая частота дисбиотического микробиома при пролиферативном фенотипе (5,3%) также не противоречила данным о росте ПЭ на фоне повышенного удельного веса Lactobacillus, Bifidobacterium, Streptococcus и Alteromonas при дефиците представителей Pseudomonas, Sphingomonas, Enterobacter [31].

Полагаем, рассуждения об участии в чрезмерном пролиферативном процессе в тканях патогенных бактерий могут оставаться спорными до выяснения вопроса о возможной персистенции в эндометрии вирусных инфектов, особенно при выявлении микрополипов.

Воспалительную основу пролиферативных процессов в группе с ХЭ (ПЭ - 28,6%, очаговая гиперплазия эндометрия - 28,6%) доказывает положительная экспрессия в тканях маркера CD138+ и гиперэкспрессия Ki-67 [32]. Потенциальными механизмами развития очаговой гиперплазии и ПЭ в группе с фенотипом ХЭ можно считать повреждение эпителия, нарушение регуляции клеточного деления с экспрессией генов, определяющих избыточный пролиферативный потенциал, угнетение апоптоза при инфильтрации ткани популяциями иммунных клеток, синтезирующих провоспалительные цитокины [33]. Вероятно, триггером пролиферативных изменений выступает длительное присутствие в эндометрии продуктов тканевой деструкции даже после элиминации микробных агентов, активирующих маркеры врожденного иммунного ответа [1]. Участие микробиоты и иммунных воспалительных сетей в генезе очаговой гиперплазии эндометрия предлагается рассматривать через повышение активности фермента β-глюкуронидазы в присутствии определенных бактерий с последующим развитием локальной гиперэстрогении [34]. Роль локального оксидативного стресса в генезе ХЭ с пролиферативным потенциалом (ПЭ, очаговая гиперплазия эндометрия) подтверждает некоторое снижение экспрессии NRF2. Выраженное снижение антиоксидантного потенциала клеток рассматривают как звено патогенеза простой и комплексной гиперплазии эндометрия [35]. Вероятно, развитие практически у половины женщин с ХЭ пролиферативных изменений было сопряжено с геномной нестабильностью эпителия эндометрия, зависящей от регулируемых факторов транскрипции микробиоты и/или эпигенетических факторов. Негативное влияние на гомеостаз клеток эндометрия прооксидантной среды связывают с дисбиотическим типом микробиоты [25]. Нарушение имплантации при пролиферативном фенотипе обусловлено иммуномодулирующей активностью GM-CSF на фоне гиперэкспрессии TNFα, СXCL16 и снижения уровней IL-10, NRF2 [36].

Заключение

Морфологическая картина ХЭ иммуногистохимически подтверждена у 57,1% бесплодных женщин (с пролиферативным гистотипом - у 33,3%). Показана неоднородность молекулярных фенотипов эндометрия внутри каждой группы (с ТПБ, ХЭ, НГЭ). Выявленные особенности доказывают сложные молекулярные взаимодействия микробиоты (лактобациллярный, смешанный и дисбиотичный типы) и иммунокомпетентных медиаторов (цитокинов, хемокинов, факторов роста), предиктивные в отношении вероятности имплантации бластоцисты. Очевидно, нарушение децидуальной трансформации изменяет экспрессию генов, участвующих в ряде процессов: регуляции клеточного цикла, реакции на стероидные гормоны и окислительный стресс, передаче сигналов цитокинов и хемокинов, иммунной модуляции.
