Taxonomic profile of the endometrial microbiome in chronic endometritis
AbstractThe aim of the study to determine the taxonomic profile of the endometrial microbiome in chronic endometritis and identify the most significant combinations of various infections.
Design: observational, cohort, closed research.
Material and methods. 88 patients of reproductive age with infertility were examined: group I (n=56) – the main one, patients with infertility on the background of chronic endometritis, group II (n=32) – control, patients without chronic endometritis. The examination included anamnestic data, patient complaints, general anamnesis and gynecological status, a comprehensive microbiological study of the microbiome and virome of the endometrium and the vaginal discharge.
Results. There was no significant difference in species composition in the endometrial microbiome between the examined groups, and the degree of contamination in most patients did not exceed 104 GE/ml. Herpesviruses were represented by HSV and CMV and were significantly more common in group I – in 19.64% of patients than in group II – in 9.38% of patients. Comparative characteristics of the species composition of the microbiome of the endometrium and vagina showed that in endometrial biopsies the degree of contamination was lower and for the most part did not exceed 104, whereas in the vagina the titer was 107–108, while the species composition had practically no differences. However, in some patients, endometrial biopsies revealed Ureaplasma urealyticum, Mycoplasma hominis, which were absent in the vagina.
Conclusion. The microbiome of the endometrium is represented by conditionally pathogenic flora: Enterobacteriaceae spp., Streptococcus spp., Gardnerella vaginalis, Ureaplasma spp. with a low degree of contamination, while in patients with chronic endometritis Lactobacillus spp. in the uterine cavity, they were detected 3 times less often.

Полученные данные свидетельствуют о приваривании нормоценоза в обеих группах без статистической разницы, p=0,126. Между другими показателями мазка также не выявлена статистическая разница.
Культуральное исследование биоптатов эндометрия на условно-патогенную флору показало, что стерильные образцы были у 38 (67,86%) пациенток I группы и у 16 (50,00%) пациенток II группы. При этом нами отмечено, что Lactobacillus spp. дали рост у 10 (17,86%) пациенток I группы и у 13 (40,63%) пациенток II группы (p<0,05). Степень обсемененности биоптатов в I группе составила 3,79×104± 1,87×105 КОЕ/мл (Me=0,00), во II группе - 9,85×104± 2,95×105 КОЕ/мл (Me=0,00), p<0,05. Условно-патогенная флора в обеих группах была представлена Enterobacteriaceae spp., Streptococcus spp., Staphylococcus spp., Gardne- rella vaginalis, Ureaplasma spp., Mycoplasma hominis, Candida albicans, без достоверных отличий в видовом составе и частоте обнаружения (р>0,05). Концентрация условно-патогенной флоры варьировала от 102 до 105 КОЕ/мл и в большинстве своем не превышала клинически значимые нормы (103 КОЕ/мл).
ПЦР-исследование отделяемого влагалища и биоптатов полости матки показало отсутствие ИППП в обеих группах (рис. 5).

У пациенток с хроническим эндометритом Lactobacillus spp. в полости матки выявлялись в 3 раза реже. Значимая разница в видовом составе и частоте выявления условно-патогенной флоры при ПЦР-исследовании также отсутствовала, а степень обсемененности в среднем составила 104 ГЭ/мл (табл. 2).

Герпесвирусы в I группе встречались значительно чаще (у 19,64%), чем во II группе (у 9,38%), p<0,05 (рис. 5). При этом в I группе были выявлены более высокие значения колониеобразующих единиц Lactobacillus spp., Enterobacteriaceae spp., Streptococcus spp., Gardnerella vaginalis, Ureaplasma spp. и Herpes simplex virus (табл. 2).
В составе микробиома влагалища у всех пациенток доминировали Lactobacillus spp. в высоком титре (107- 108 ГЭ/мл). У пациенток I группы значимо чаще встречались факультативно-анаэробная условно-патогенная флора кишечной группы (48,15%), стафилококки (42,59%), стрептококки (29,09%), представленные в низком титре (103 ГЭ/мл). Ureaplasma parvum и Mycoplasma hominis были выявлены только у пациенток I группы (21,43 и 5,36% соответственно), а титр не превышал 103 ГЭ/мл. Gardnerella vaginalis, Atopobium vaginae и Candida albicans встречались одинаково часто в обеих группах в клинически значимых титрах (103-106 ГЭ/мл). Виром был представлен герпесвирусами в I и II группах с наиболее частым выявлением их у пациенток с ХЭ, при этом титр не превышал 104 ГЭ/мл (рис. 6).

Сравнительная характеристика видового состава микробиома эндометрия и влагалища в обследуемых группах показала, что в биоптатах эндометрия степень обсемененности была ниже (102-106 ГЭ/мл), чем во влагалище (107- 108 ГЭ/мл), при этом видовой состав практически не имел отличий (рис. 7).

Однако у некоторых пациенток в биоптатах эндометрия были выявлены Ureaplasma urealiticum, Mycoplasma hominis, которые отсутствовали во влагалище (см. рис. 7).
Заключение
Оценка клинико-анамнестических данных показала: пациентки с ХЭ предъявляли жалобы в 3 раза чаще на болезненные, обильные менструации и межменструальные кровянистые выделения. Длительность бесплодия в I группе была в 2,5 раза выше, чем во II (р=0,000), как и количество неудачных попыток ЭКО.
По результатам микроскопии влагалищных мазков значимых отличий не было (р>0,05), что можно объяснить низкой специфичностью данного исследования.
При культуральном исследовании биоптатов эндометрия у пациенток с ХЭ колонизация Lactobacillus spp. была достоверно реже (17,86%), по сравнению с колонизационной резистентностью у обследуемых без ХЭ (40,63%), p<0,05, при этом степень обсемененности имела схожие значения (104 КОЕ/мл).
Микробиом эндометрия представлен условно-патогенной флорой: Enterobacteriaceae spp., Streptococcus spp., Gardnerella vaginalis, Ureaplasma spp. с низкой степенью обсемененности, при этом у пациенток с ХЭ Lactobacillus spp. в эндометрии выявлялись в 3 раза реже.
