Iron deficiency anaemia and liver condition in pregnant women: correlation connection
AbstractThe aim of the study was to evaluate the anaemia markers and the level of hepatic tests in pregnant women with anaemia in dynamics, and to conduct correlation analysis to optimize the obstetrics management.
Material and methods. There were examined 2253 pregnant women, of whom 171 (7.6%) were diagnosed with anaemia. The blood tests including RBC, hemoglobin level, hematocrit, MCH, ALT, AST, bilirubin levels were evaluated at 6–12th weeks, 20th week, and 30th week of pregnancy. The ferritin and transferrin levels, and common iron binding ability were determined at the 30th week of pregnancy. All women in the study group were treated with iron of up to 4 weeks during early gestation.
Design: observational, cohort, closed research.
Results. Against the background of anti-anemic therapy for 4 weeks, there was a significant increase in hemoglobin, hematocrit, RBC, MCH at 20th weeks of gestation. When comparing the medians of the studying haematological tests conducted at the 30th week, significant differences were achieved only in terms of MCH, while the indicators of hemoglobin, hematocrit, and RBC remained at the same level. The levels of ALT, AST and bilirubin differed significantly in all stages of pregnancy (р=0,000). The ferritin level at the 30th week was on average lower than the normal rate, and was 12.957±6.843µg/l. Correlation analyses revealed moderate correlation between RBC, Hb and Ht (R from 0.48 to 0.61), ALT and AST (r=0.63), which persisted by the 20th week of pregnancy. There were also a moderate correlation between bilirubin and Hb (r=0.25) and weak correlation between bilirubin and Ht (r=0.20) at 6–12 weeks of gestation. There was a moderate correlation between ALT and AST (r=0.54), as well as between Hb and MCH (r=0.276). There were a weak correlation between transferrin and RBC (r=-0.171), and between bilirubin and ferritin (r=0.18), and between bilirubin and the common sideropexy power (r=-0.15).
Conclusion. In case of detection of anaemia in pregnant women, it is advisable to recommend the use of hepatoprotectors against the background of iron therapy. The use of such integrated approach will also improve the conditions of pregnant women, and contribute to the accumulation of iron in hepatic cells. If initial anaemia is detected, iron preparations should be taken throughout pregnancy, which contribute as blood management, as prevention of severe anaemia in labor. Initial hemoglobin levels 125 g/l or less may be considered as a criterion for pre-latent iron deficiency in pregnant women. It is recommended to determine the iron and ferritin levels in trimester I.

Анализ показал, что минимальный уровень железа в 30 нед беременности составил 2,8 мкмоль/л при его нормальных показателях 9-30,4 мкмоль/л. Даже медиана уровня железа имела низкие значения - 9,4 мкмоль/л. Как известно, железо входит в состав функциональных групп белков, которые транспортируют кислород, оно участвует в клеточном обмене. Основная часть железа находится в клетках печени, селезенки и др. [22, 23].
Ферритин и его агрегированная форма - гемосидерин играют роль резервуаров для железа, депонируют его. Минимальный его показатель среди обследованных составил 4,9 мкг/л, что практически в 3 раза меньше нижнего его референтного интервала (норма 15-150 мкг/л), и максимальный уровень (51,0 мкг/л) в 3 раза ниже верхней границы референтного интервала [23]. Ферритин является "золотым стандартом" оценки количества железа, которое запасено организмом: количество ферритина прямо пропорционально накоплению железа в макрофагах и гепатоцитах [21].
Трансферрин транспортирует железо по организму, и поэтому его уровень определяет возможность доставки железа в клетки [23]. Этот показатель среди обследованных беременных был в пределах нормы (норма 2-3,6 г/л).
Важным показателем является ОЖСС крови, которая коррелирует с уровнем трансферрина. Если наблюдается дефицит железа, то уровень ОЖСС будет повышен (норма 27,8- 53,7 мкмоль/л) [21]. Минимум ОЖСС крови среди обследованных был равен 17,9 мкмоль/л и максимум - 123,0 мкмоль/л.
Нами проведено исследование корреляционных связей путем определения коэффициента корреляции Спирмена (r). Имеется умеренная корреляция в 6-12 нед беременности между уровнями гематокрита и эритроцитов (r=0,61); уровнями гематокрита и гемоглобина (r=0,58); уровнями гемоглобина и эритроцитов (r=0,48). Определена умеренная корреляция в этом же сроке между показателями АЛТ и АСТ: r=0,63, а также слабая корреляция между уровнями билирубина и гемоглобина (r=0,25) и билирубина и гематокрита (r=0,20).
В сроке беременности 20 нед сохранялась умеренная корреляция между уровнями гематокрита и эритроцитов (r=0,54); уровнями гематокрита и гемоглобина (r=0,42); уровнями гемоглобина и эритроцитов (r=0,32); а также показателями АЛТ и АСТ (r=0,58).
В 30 нед беременности корреляция между АЛТ и АСТ составила r=0,54. На этом фоне ранговая корреляция Спирмена между гемоглобином и МCH в 30 нед беременности была умеренной и составила r=0,276; между уровнями трансферрина и эритроцитов - слабой: r=-0,171. Выявлена слабая корреляция в 30 нед беременности между показателями билирубина и ферритина: r=0,18; билирубина и ОЖСС крови: r=-0,15.
Заключение
Представленные корреляционные связи позволяют сделать следующие выводы: умеренная корреляция в 6-12 и 20 нед беременности между уровнями гематокрита, гемоглобина и эритроцитов показывает связь между этими показателями и их линейную зависимость. В 30 нед беременности данная связь отсутствовала. Однако отмечена умеренная корреляция между уровнем гемоглобина и МCH. Кроме этого, была выявлена слабая связь между уровнями трансферрина и эритроцитов.
В течение всего наблюдения между показателями АЛТ и АСТ имелась умеренная корреляция, при этом связь АЛТ и АСТ с гемоглобином, эритроцитами, гематокритом, МCH, а также с железом, трансферрином, ферритином и ОЖСС крови отсутствовала. Однако между уровнем билирубина в 6-12 нед беременности имелась слабая и умеренная корреляция с гематокритом и гемоглобином соответственно, как и между уровнем билирубина и ферритина, а также билирубина и ОЖСС крови. Это позволяет нам рекомендовать назначение гепатопротекторов у пациенток с дефицитом железа во время беременности на фоне терапии препаратами железа. Комплексный подход улучшит не только состояние беременных, но и будет способствовать накоплению железа, основная часть которого находится в клетках печени.
Повышение уровня эритроцитов и гемоглобина от 6-12 до 20 нед беременности, обусловленное приемом железосодержащих препаратов, к 30-й неделе при отсутствии лечения нивелируется. Отмеченное снижение в динамике к 30-й неделе беременности гематокрита и MCH, а также низкие показатели железа и ферритина в сыворотке крови показывают необходимость приема препаратов железа у беременных с исходной анемией на протяжении всего периода беременности. Данный подход: сочетанное применение железосодержащих препаратов и гепатопротекторов будет способствовать менеджменту крови и профилактике возникновения тяжелых анемий в родах при физиологических родах и оперативном родоразрешении, что находит отражение и в других исследованиях [24].
Учитывая полученные низкие показатели содержания железа и ферритина в сыворотке крови в 30 нед беременности, необходимо проводить их определение при постановке беременной на диспансерный учет в I триместре, что совпадает с данными других исследователей [25] и будет способствовать профилактике железодефицитных состояний при беременности.
