Clinical observation of missed invasive cervical cancer during pregnancy

Abstract

Cervical cancer (CC) is an urgent problem in gynecological oncology, occupying a leading place among malignant neoplasms associated with pregnancy and first place among malignant neoplasms associated with pregnancy. The frequency of detection of cervical cancer according to the literature ranges from 1 to 13 cases per 10 000 pregnancies, including the postpartum period, and 1 per 1000–2500 births. The average age of patients with cervical cancer in combination with pregnancy is 30 years, and in the age group of 25–34 years it ranks first in the structure of the incidence of malignant neoplasms, amounting to 22.5%. Pregnancy, due to the hormonal changes that occur, primarily due to hyperestrogenism, active angiogenesis, as well as elements of immunosuppression, can contribute to the progression of oncopathology. On the other hand, a malignant tumor, examination and treatment (both previously and during pregnancy) can have an adverse effect on the development of the fetus. Thanks to the introduction of modern and accurate diagnostic methods and mass screening of women, the incidence and mortality from cervical cancer have decreased significantly in recent years.

The presented article describes a clinical observation of invasive cervical cancer with characteristic symptoms missed in the 1st level hospital in a patient of reproductive age at the beginning of the 2nd trimester of pregnancy. Taking into account the duration of pregnancy, stage of the oncological process, presence of severe bleeding and the optimally chosen tactics for managing a patient with cervical cancer, a relatively favorable outcome for life and health was obtained.
