Hysteroscopic predictors of recurrent endometrial polyps in reproductive age

Abstract

Endometrial polyps are a common condition in women of reproductive age, but their pathogenesis remains unclear. Despite their often asymptomatic course, polyps are associated with menstrual irregularities and infertility. The recurrence rate after polypectomy, according to different authors is 7.8–46%. The factors influencing the recurrence of polyps are insufficiently studied. There is a need to develop hysteroscopic predictors to predict the personal risk of recurrence.

The aim of the study was to identify hysteroscopic predictors of endometrial polyp recurrence in patients of reproductive age.

Material and methods. 96 women with primary endometrial polyps [median age 37.58 years (Q1–Q3: 33.543–40.242)] and 94 women with endometrial polyp recurrence [median age 36 years (Q1–Q3: 34.30–37.99] were included in a prospective cohort comparative open study. Anamnesis, anthropometric data were collected, sonographic examination of pelvic organs and hysteroscopy of the uterine cavity were performed. Statistical analysis with calculation of odds ratio was carried out.

Results. Significant (p<0.05) hysteroscopic predictors of recurrence of endometrial polyps are: diffuse hyperemia (OR 5.413; 95% CI 2.332–12.561), vascularization (OR 5.394; 95% CI 2.221–13.100), micropolyposis (OR 4.859; 95% CI 1.878–12.575), stromal edema (OR 3.077; 95% CI 1.148–8.264), focal hyperemia (OR 2.456; 95% CI 1.265– 4.766) and wide polyp base (OR 2.060; 95% CI 1.103–3.850).

Conclusion. Chronic endometritis may contribute to recurrence of endometrial polyps.
