Outpatient and inpatient hysteroscopy. Comparative analysis of indications, visualization and histological confirmation

Abstract

The aim of the research - to conduct a comparative analysis of visualization and histological confirmation in women with uterine and endometrial pathology during outpatient and inpatient hysteroscopy.

Tasks: 1) to compare the data of hysteroscopic visualization of the pathology of the uterus and endometrium obtained with office hysteroscopy and stationary hysteroscopy; 2) to compare the histological results obtained during office and stationary hysteroscopy.

Material and methods. A retrospective analysis of patients with pathology and endometrium who underwent outpatient hysteroscopy at the State Medical Institution KGKB No. 4 and inpatient institutions in Kemerovo in the period from 2018 to 2020 (according to medical documentation) was carried out. The study included 600 women: Group 1 - 300 women who underwent outpatient (office) hysteroscopy (OGS) and group 2 - 300 patients who received inpatient stationary hysteroscopy (SHS). Confirmation of endometrial pathology in women of the two groups was carried out by manual vacuum aspiration of the uterine cavity (MVA).

Results. Indications, visualization and histological confirmation during the OGS and GHS did not differ in most nosologies. However, for women with infertility, invasive intervention was more often performed in a hospital (p=0.035). OGS more often revealed adenomyosis and deformity of the uterine cavity. Histological confirmation of the pathology of the uterus and endometrium in OGS was comparable with SGS, with the exception of the glandular fibrotic polyp of the endometrium. Visualization and histological confirmation of mono and combined pathology of the uterus and endometrium in OGS was more common at the age of 45-59 years, during SGS - in women older than 60 years (p=0.001).

Conclusion. Thus, the absence of significant differences in indications, imaging and histological results in patients referred to OGS and SHS proves the equivalence of outpatient and inpatient hysteroscopy. However, OGS has advantages in detecting adeomyosis and deformity of the uterine cavity. The preferred age for its implementation is 45-59 years.
