Effectiveness of local hormone therapy depending of duration postmenopausal, mineral bone density, level estrogen receptor in patients with sclerosing lichen vulva

Abstract

The problem of dystrophic diseases and vulvar cancer still remains unresolved. Most of the researchers to the main etiological causes of these diseases, include disorders in the system of neuroendocrine homeostasis that occur in postmenopausal women. Along with this, a reduction in the level of estrogen receptors in the tissue of the vulva, which forms in postmenopause, is also of some importance. Knowledge of the receptor status of the vulvar tissue in this pathology in postmenopausal patients allows for a differential construction of a hormonal therapy program. For an adequate realization of the hormonal effect, not only the presence of a hormone in peripheral blood is necessary, but also a sufficient level in the tissues - targets of specific receptors. The response of tissues to the effects of steroid hormones is determined by the concentration of receptors, their structure and the properties of hormones. The local effect of steroid hormones is realized by passive diffusion into the cells of the body. The priority role in the choice of therapy for atrophic disorders, caused by a decrease in gonad function, belongs to estriol-containing drugs. This is due to the specific local sensitivity of the tissues of the lower parts of the urogenital tract to estriol. There are several criteria that determine the indications and possible prognosis when performing hormone therapy in patients with sclerotic lichen. These include bone mineral density and estrogen-receptor level in the vulvar tissue, depending on the length of the postmenopausal period.
