Determination of signaling lymph nodes in patients with vulvar cancer

Abstract

Aim. To assess the efficacy of sentinel Lymph node procedure for patients with vulvar cancer.

Material and methods. 39 patients with FIGO I stage squamous vulvar cancer were included in the study. Regional lymph nodes were assessed using both ultrasound and clinical palpation, seems benign at the moment of investigation. Multifocal tumor growth was an exclusion criterion. For scintigraphy radioactive tracer 99mTc was used. During surgery SLN were detected with gamma probe Gamma Finder II (W.O.M., Germany) in standard setting.

Results. One or more SLNs were detected by scintigraphy in 97% (n=38). Bilateral detection was achieved in 21% (n=8). Intraoperative detection rate using gamma probe was 100%. Frozen section analysis found metastasis in 23.6% (n=9) and shown sensitivity 94.7%. All patients with lymph node involvement had next tumor characteristic: diameter more than 2 cm, invasion more than 5 mm. Recurrence rate in lymph nodes was 2.6%.

Conclusion. SLN procedure is highly effective and can be recommended for routine use.
