Benign endometrial processes at postmenopausal age. Effectiveness of microwave endometrial ablation

Abstract

The aim of the study was to evaluate effectiveness of microwave endometrial ablation (MEA) in postmeno­pausal women with benign endometrial processes using modern diagnostic methods.

Material and methods. The authors examined 80 postmenopausal women with benign endometrial pro­cesses at the ages from 52 to 80 years. In all patients before and after MEA, in several dates of hospitalization and different medical history (from 14 days to 3 years) separate diagnostic curettage of uterine cavity mucous walls under hysteroscopy control was carried for benign endometrial processes. All patients with diagnostic goal before MEA and in in the course of patient supervision in 1, 3, 6, 12, 15 months ultrasound investigation were carried out in terms of two-dimensional and three-dimensional echocardiography (2D ultrasound and 3D ultrasound). 3D-ultrasound gave the opportunity to follow up changes in uterine cavity, as well as Asherman syndrome forming stages.

Results. Perfect clinical effect after MEA was noted in 77 patients. In 3 patients with atypical endometrial hyperplasia removal of the uterus with appendages was performed.

Conclusion. MEA is safe and easily feasible method of benign endometrial processes in postmenopausal women treatment. Before MEA it makes sense to carry out control hysteroscopy, endometriumctomy of uterine cavity mucous walls with histopathological examination of tissue in case of pathological changes in endometrium presence. MEA efficacy endpoints are: absence of blood demergers from genital tract; thin endometrial echo (up to 4 mm) or Asherman syndrome formation; absence of blood flow in arcuate, radial and basal uterus arteries on CDI.
